This procedure is applied especially in the presence of clinical findings such as infertility, pain, and decreased testicular volume.

In response to the question of how varicocele surgery is performed, it is stated that microscopic surgical techniques are the most frequently preferred method today. Through a small incision in the groin area, the enlarged veins are tied off and healthy blood flow is restored.

The recovery process after varicocele surgery is generally fast, and patients can return to their daily lives in a short time. Mild pain and swelling may be seen after the operation, but these conditions are mostly temporary and are controlled with appropriate care.

When the success rate and effects of varicocele surgery are evaluated, improvement in sperm quality and reproductive functions is frequently observed. Early diagnosis and appropriate surgical intervention play an important role in preventing long-term complications.

Things You Should Know

Information

Definition of the Surgery

Varicocele surgery is the procedure of normalizing blood flow by surgically ligating the enlarged veins around the testicle (varicocele).

Purpose of the Surgery

To correct blood flow in the testicles, increase sperm quality, and relieve pain caused by varicocele.

Methods of Application

It can be performed with microscopic varicocelectomy (microsurgery), laparoscopic surgery, or open surgery methods.

Most Frequently Used Method

Microscopic subinguinal varicocelectomy; what kind of change does it create in the body?

In order for the testicles, which are the factories of the male reproductive system, to fulfill their functions perfectly, they must be constantly nourished with clean blood and the used dirty blood must be removed rapidly. Fresh and oxygen-rich blood pumped from the heart reaches the testicles through the arteries. The blood that has been used in the tissues, has low oxygen, is warmed, and filled with metabolic waste then sets off back toward the heart through the veins (pampiniform plexus). Inside a healthy vascular structure, there are many one-way small valves that prevent the blood climbing upward against gravity from falling backward. These valves work like check valves, guaranteeing that the blood flows only in one direction toward the heart.

However, in some situations, the system collapses as a result of these valves being congenitally weak, their structural integrity being impaired, or the hydrostatic pressure inside the vein increasing excessively. When the valves fail to do their job, the dirty blood that should move upward begins to flow backward downward, that is, toward the scrotum, with the effect of gravity. This backward escape process, called “reflux” in medicine, increases the fluid load inside the veins to enormous levels. The flexible vessel walls, exposed to constant pressure, become tired over time, dilate, lengthen, and turn into tortuous bundles inside the scrotum. This venous enlargement is seen in a considerable part of the general male population. In fact, it appears as the main underlying problem in a large proportion of men seeking medical support due to inability to have children. In men who have previously had children naturally but experience difficulty when trying for a second or third child, the frequency of this problem reaches much more striking dimensions.

Why is the vast majority of varicocele cases seen on the left side?

In clinical evaluations and diagnostic processes, it is seen that this venous enlargement occurs on the left side of the body, that is, in the left testicle, in more than ninety percent of cases. The main reason behind this situation, which may seem surprising at first glance, is hidden entirely in the asymmetric and rather interesting anatomical structure of the human body. The veins carrying the dirty blood from the right and left testicles must follow completely different routes while progressing upward inside the abdominal cavity.

The vein coming from the testicle on the right side completes its journey quite easily. It joins the inferior vena cava, the largest vein in the abdomen that goes directly to the heart, at a narrow and inclined angle. This joining is like a vehicle smoothly entering a highway without slowing down; the blood flow is uninterrupted and comfortable. However, the situation is not nearly as easy for the vein of the left testicle. The left vein cannot directly enter the main highway; instead, it must connect to the vein coming from the left kidney. Moreover, due to anatomical necessity, this connection occurs at a full 90-degree vertical angle, like a “T-junction.” According to the rules of fluid dynamics, blood trying to flow at a right angle into another vein creates serious resistance and high backward pressure.

In addition to this anatomical disadvantage, because the left testicle is anatomically located slightly lower than the right, the left vein is approximately eight to ten centimeters longer. The fact that the blood has to climb this extra distance against gravity further increases the pressure inside the vein. In some rare situations, a mechanical compression called the “Nutcracker Phenomenon” can also be added to the picture. The left renal vein, which passes between two major arteries in the abdomen, may remain under pressure like a walnut squeezed between the arms of a nutcracker. This compression creates a tremendous shock wave backward and renders the valves in the testicular veins completely dysfunctional. If this problem is seen only on the right side, this is a very unusual condition, and the presence of a different mass externally compressing the veins on the back wall of the abdomen must absolutely be investigated with detailed radiological examinations.

How do enlarged varicocele veins impair sperm quality and testicular structure?

The production process of sperm (spermatogenesis), the male reproductive cell, is one of the most sensitive and complex biological mechanisms in the human body. For this production process to function perfectly, the testicles must be in a special environment isolated from the rest of the body. While our internal body temperature is around 37 degrees on average, in order for the testicles to produce ideal sperm, the temperature needs to remain between 33 and 34 degrees. The real evolutionary purpose of the scrotum being outside the body is to provide this natural cooling. The healthy venous network surrounding the testicles functions like a tremendous radiator or air conditioning system that absorbs the heat of the warm blood coming from the arteries and cools it before it enters the testicle.

When varicocele develops, this perfect climate control system collapses. The warm body blood that pools in the enlarged veins and constantly leaks backward creates a thermal blanket effect around the testicle. Even an increase of only half a degree in intratesticular temperature stops the functioning of vital enzymes responsible for cell division and sperm production. Serious breaks and damage occur in the genetic material (DNA) of the sperm that are attempted to be produced under this thermal stress. The fertilization capacity of a sperm with damaged DNA decreases dramatically; even if fertilization occurs, the risk of pregnancy ending in miscarriage at an early stage increases significantly.

The situation is not limited only to the increase in heat. The slowing and pooling of blood in the veins causes the testicular tissue to receive insufficient fresh oxygen (hypoxia). Cells deprived of oxygen cannot fulfill their functions. In addition, metabolic waste such as cortisol secreted from the left kidney and adrenal gland descends directly toward the testicle in a reverse flow because the valves are defective. These chemicals cause the microscopic vessels in the testicle to constrict and the damage to deepen further. When all these adverse conditions combine, oxidative stress, which we can describe as cellular rusting, begins in the cells. Harmful free radicals destroy the outer membrane of the sperm, weakening their tail movements and depriving them of their ability to swim forward. This chronic destruction lasting for years eventually also exhausts the cells producing testosterone, the male hormone, in the testicle, potentially paving the way for hormonal imbalances in the long term.

The main factors playing a role in the formation of damage are as follows:

Thermal stress

Oxygen deficiency

Oxidative stress

Exposure to toxic substances

Hormonal disruption

Contact us now to get detailed information about treatments and to make an appointment!

 Contact via WhatsApp

What are the symptoms of varicocele in daily life and how is a definitive diagnosis made?

In the majority of patients, varicocele can progress completely silently and insidiously for years without causing any symptoms. Many men who do not feel any pain or soreness in daily life learn about this condition by chance only when they enter the medical investigation process because of inability to have children. However, the picture does not always remain this silent. In patients who show symptoms, the most typical complaint is a feeling of heaviness and fullness in the scrotum that appears in the later hours of the day, especially when standing for a long time or performing heavy physical effort. This feeling is most often not a sharp pain but a tiring and dull ache that pulls downward. Some patients may notice with their hands the tortuous, worm-like bundles of veins on the scrotum while showering or during personal hygiene. Rarely, a visible shrinkage or asymmetry in the testicle with varicocele compared to the other one may attract the patient’s attention.

For a definitive and reliable diagnostic process, the gold standard method is a detailed physical examination performed by an experienced physician. No matter how advanced ultrasonography devices are, they cannot replace a hand examination performed with the correct technique. For the examination to yield the correct result, the clinical environment must be warm; in a cold room, the scrotal muscles may contract and hide the veins. The patient must be examined while standing, because in the lying position the effect of gravity disappears and the enlarged veins instantly empty and disappear. While the physician controls the veins as the patient stands relaxed, he then asks the patient to take a deep breath and strain forcefully by tightening the abdominal muscles (Valsalva maneuver). This straining increases intra-abdominal pressure, pushing blood downward, and the blood escaping from the defective valves immediately distends the veins. Color Doppler Ultrasonography should only be used as a supportive tool in very overweight patients where physical examination is difficult, in situations where the scrotal structure does not allow examination, or to clarify clinical suspicions. In the process of infertility investigation, only the diagnosis of varicocele is not sufficient; at least two detailed semen analyses (spermiograms) given at certain intervals must absolutely be requested from the patient, and the number, motility, and structural characteristics of sperm cells must be carefully examined.

The common physical symptoms are as follows:

Feeling of heaviness in the scrotum

Dull aching

Visible venous enlargement

Asymmetry in the scrotum

Shrinkage in the testicle

How are varicocele stages classified according to clinical examination findings?

After the diagnosis is clarified, varicocele cases are graded according to an internationally accepted system in order to determine the severity of the disease, the extent of the damage it causes to the tissues, and to create the correct treatment strategy. This grading is based entirely on findings from hand examination while the patient is standing and is the most decisive guide regarding the necessity of treatment. The progression of the disease is evaluated in four different stages.

In the initial stage called the subclinical stage, there is no venous enlargement visible from the outside or palpable by the physician’s hand while the patient is standing, straining, or coughing. The problem at this stage can only be detected by measuring the backward flow of blood using the sensitive probes of the ultrasonography device. Clinically, this is the mildest picture.

In the first stage, the disease has begun to become slightly more evident. While the patient is standing comfortably, the veins still cannot be felt. However, when the physician asks the patient to take a deep breath and strain strongly, the blood rushes downward with the increase in intra-abdominal pressure, and the veins that enlarge at that moment are clearly felt at the physician’s fingertips. When the condition progresses to the second stage, the picture becomes even easier to notice. While the patient is only standing, without the need for any straining, coughing, or tightening of the abdominal muscles, the enlarged, distended venous bundles can be palpated easily during the examination.

The third stage represents the most advanced and severe stage the disease can reach. At this stage, touching may not even be necessary for diagnosis. When looking at the scrotal area from the outside while the patient is standing, excessively enlarged and prominent vascular networks twisted like a “bag of worms” or a “package of spaghetti” can be seen very clearly under the skin with the naked eye. Generally, the most severe damage is observed in patients at this stage.

The grading steps are as follows:

Subclinical

First stage

Second stage

Third stage

Should a surgical treatment decision absolutely be made for every diagnosed varicocele case?

A very common misconception in society is the belief that every man diagnosed with varicocele should immediately lie down on the operating table without delay. However, current medical principles and international guidelines adopt an extremely selective and protective approach in this regard. The decision for surgery is never made for the purpose of cosmetic correction or simply because “there is a large vein there.” In order for a surgical intervention to be planned, the varicocele must be causing concrete, demonstrable, and clinically meaningful harm to the patient’s health, quality of life, or reproductive potential.

The most fundamental indication for treatment arises when couples cannot have children despite regular and unprotected intercourse. If the man has a first, second, or third stage varicocele detectable on manual examination and, at the same time, semen analyses show reduced sperm count, slowed motility, or structural (morphological) abnormalities, surgical treatment should absolutely be brought to the agenda. Of course, for this decision to be healthy, the reproductive health of the female partner must also be evaluated simultaneously and confirmed to be normal or treatable. In single men who are not yet married, are at an early age for family planning, but have severe varicocele and significant decreases in semen analyses, treatment is also recommended in order to preserve the future capacity of the testicles.

Apart from all this, severe physical complaints caused by varicocele may also be a reason for surgery. If the patient continues to suffer from severe and persistent testicular pain that lowers quality of life and interferes with work life despite using simple painkillers, wearing supportive cotton underwear, and avoiding heavy physical activities, surgery is considered as an option. In addition, in couples who will benefit from assisted reproductive techniques (IVF, microinjection), if sperm DNA damage is very high in the man, varicocele treatment may be applied beforehand to increase the chance of embryo implantation and maximize pregnancy success. However, it should be emphasized again that in cases of subclinical varicocele seen incidentally only on ultrasonography and not palpable by hand, routine surgical intervention is definitely not recommended if there is no obvious infertility problem.

The main situations supporting the decision for surgery are as follows:

Presence of infertility

Decrease in semen parameters

Resistant pain affecting daily life

Progressive testicular atrophy

High sperm DNA damage

What path should be followed in varicocele detected in adolescents?

Adolescence is an extremely critical period of time in which hormonal storms are experienced in the human body, rapid physical development is seen, and the reproductive system begins to mature. Varicocele can be detected in approximately fifteen percent of boys in this period. The venous enlargements detected in adolescents are generally completely painless and silent; young people most often notice this condition by chance or due to the attention of their families. Making a treatment decision for patients in this age group requires a much more delicate balance compared to adults. There is a fine line between not subjecting a growing adolescent to unnecessary surgical trauma and not putting his future chance of fatherhood at risk:

For this reason, the first choice in adolescent varicoceles is always close follow-up and a “wait-and-see” strategy. The young patient is called for control at certain intervals. However, there are red lines during follow-up that change the picture and require urgent intervention. If the testicle on the side with varicocele has slowed in growth rate compared to the healthy testicle on the other side and there is a volume difference of more than two milliliters or more than ten to twenty percent between them (this condition is called testicular atrophy), this is an absolute surgical warning. The loss of size in the testicle is the most concrete proof that the tissue is beginning to be permanently damaged. In addition, if an abnormal softening in testicular consistency is observed or if serious reductions in sperm values are detected in semen analyses performed in youths over the age of 16 compared to their age group, treatment planning should be initiated without delay.

The factors leading to a decision for surgery are as follows:

Difference in testicular size

Softening in testicular consistency

Abnormality in sperm count

Complaint of pain

Contact us now to get detailed information about treatments and to make an appointment!

 Contact via WhatsApp

Which is the most successful and reliable surgical method for varicocele treatment today?

The definitive and permanent treatment of varicocele in medical literature is possible only through surgical means; it is biologically impossible for a dilated vein with damaged valves to return to its former anatomical form with medication, diet, or herbal cures. In modern andrology and urology practice, the “Gold Standard” treatment method accepted worldwide, with the highest success rate and the lowest side effect profile, is Microsurgical (Microscopic) Subinguinal Varicocelectomy. The basic philosophy of this operation is extremely clear and at the same time requires mastery: to identify and ligate all faulty veins that poison the testicle, heat it, and cause blood pooling, thereby disabling them; while doing this, to preserve with millimetric precision the tiny arteries that are the life source of the testicle, the transparent lymphatic channels that carry tissue fluid, and the duct that carries sperm.

This delicate procedure is performed through a very small aesthetic incision of approximately two to three centimeters made just below the patient’s groin region. The most important element that distinguishes this surgery from standard procedures and gives it superiority is the use of special high-resolution operating microscopes during the operation. These devices magnify the surgical field enormously, between eight and fifteen times, and reveal with tremendous clarity anatomical details that the surgeon could never see with the naked eye.

The foremost advantage provided by the use of the microscope is arterial preservation. The arteries feeding the testicle can sometimes be fractions of a millimeter in diameter, as thin as a strand of hair. Accidentally ligating one of these vessels would cut off the fresh blood going to the testicular tissue and lead to shrinkage and complete death of the testicle (atrophy). Thanks to the microscope, the pulsations of these vital vessels in sync with the heartbeat are clearly seen and they are separated aside without being touched. The second critical advantage is the preservation of lymphatic vessels. Lymphatic channels are tiny transparent tubes filled with colorless fluid. If they are damaged or accidentally cut, fluid begins to accumulate in the scrotum and a troublesome complication called “hydrocele,” which can cause the testicle to swell to the size of a grapefruit, develops. Since these channels are clearly identified and preserved in microsurgery, the risk of hydrocele falls below one percent. The third and most important advantage is the identification and ligation, under the microscope, of even the very small collateral veins hidden within the muscle tissue or around the vascular sheath that are too small to be seen otherwise. In this way, the probability of recurrence of the disease in the future is almost eliminated and reduced to an incredibly low level, such as one percent.

The main advantages of microsurgery are as follows:

High success rate

Preservation of arteries

Preservation of lymphatic channels

Low probability of recurrence

Minimal risk of hydrocele

Why have other varicocele treatment methods such as conventional open surgery or laparoscopy remained in the background?

Many different methods have been tried and applied in the history of medicine for the treatment of varicocele. However, with the development of medical technology and the widespread adoption of microscopic techniques, the high risks and low success rates carried by old-generation methods have pushed them far back on the list of medical options. Conventional open surgery (Ivanissevich or Palomo methods), which was frequently applied in the past, was performed without the use of any optical magnification device, with the naked eye or very simple magnifiers. Due to the limits of the naked eye, the probability of accidentally cutting or ligating tiny arteries and transparent lymphatic channels was very high. Even worse, because small and hidden varicose veins were overlooked, the recurrence rate of the disease with the same severity a few months after surgery could reach up to fifteen percent, while the rate of hydrocele development could rise up to eight percent.

Laparoscopic varicocele surgery, on the other hand, is based on inserting a camera and long instruments into the abdomen and ligating the veins in the upper parts of the abdominal cavity. Although it may sound attractive because it is a closed surgery, it is a rather aggressive approach for varicocele. Since the peritoneum is penetrated and the area where the intestines and internal organs are located is traversed, it imposes serious complications such as bowel or major vascular injuries that can threaten life entirely unnecessarily on the patient. Moreover, because it is technically much more difficult to distinguish arteries or lymphatic channels at the microscopic level with laparoscopic instruments, the complication rates are high. The method of vascular occlusion called percutaneous embolization, performed by interventional radiologists, is the procedure of entering through the groin with a needle and leaving adhesive material or occlusive metal coils inside the vessel. Although the absence of a surgical incision seems like an advantage, the procedure has a high failure rate and recurrence of the disease through the opening of new capillaries right next to the occluded vessel is encountered very frequently.

The main disadvantages of the other methods are as follows:

Risk of intra-abdominal organ injury

High rate of disease recurrence

Frequent fluid accumulation

Serious possibility of testicular shrinkage

Failure of the procedure

What is the recovery and return-to-normal-life process like after a successful varicocele surgery?

After varicocele surgeries performed with the microsurgical method, patients are faced with a very fast, painless, and comfortable recovery process. After undergoing surgery with general anesthesia or spinal anesthesia, patients can generally be discharged and return home comfortably on the same day or after only one night of clinical observation. Since the small surgical incision in the groin area is generally closed with hidden and self-dissolving sutures, there is no trouble of later having sutures removed; the superficial closure and healing of this area takes approximately one to two weeks. Resting at home for the first few days, taking short walks, and consuming plenty of fluids is sufficient.

The most important factor that will increase the patient’s daily comfort in the postoperative period is the use of cotton and slightly tight underwear (suspensory or briefs) that support the testicles from below. This support accelerates healing by reducing the pressure that gravity will exert on the surgical area. Individuals who work at desk jobs or have professions that do not require heavy physical strength can generally return to their normal work pace within a few days. However, avoiding heavy lifting, intense fitness training, weight training, or strenuous sports activities that suddenly and severely increase intra-abdominal pressure for at least four to six weeks is an extremely critical rule to prevent damage to the healing vascular tissues.

The things to be followed during the recovery process are as follows:

Avoiding heavy lifting

Wearing supportive underwear

Using prescribed medications regularly

Taking a break from strenuous sports

Not neglecting doctor follow-up

When does improvement in sperm values and pregnancy chance increase after surgical intervention?

Many patients hope that all sperm values will return to normal as if by magic just a few days or weeks after surgery, but human biology has its own unfailing timetable. The process of a stem cell in the testicles transforming into a fully equipped, motile, and fertilization-capable mature sperm cell from scratch (the spermatogenesis cycle) takes an average of 72 to 90 days. In other words, the healing that begins in the testicles on the day of surgery will be released as fresh and healthy sperm months later. Therefore, having a test early to see the effects of the operation creates nothing but unnecessary stress and disappointment. The first follow-up semen analysis should be planned no earlier than 3 months after surgery.

Cellular recovery and the re-establishment of healthy functioning in the testicular tissue progress on a gradually increasing graph, and the values generally continue to improve until the sixth to twelfth month after surgery. Large-scale scientific studies and statistics have proven that after a successful microsurgical varicocele repair, an extraordinary and permanent improvement occurs in sperm count, motility, and morphology in approximately sixty to eighty percent of patients. The most beautiful fruit of this cellular improvement is the increase in pregnancy rates. At the end of the one-year postoperative period, the chance of couples achieving pregnancy naturally, without the need for any external artificial intervention (IVF, insemination), can rise to around forty-five percent, and even to sixty percent in some favorable clinical pictures.

Does varicocele treatment work in cases where there is no sperm at all in the semen or in those who only have chronic pain?

The condition in which no live or dead sperm cells are seen under the microscope in the semen analysis is called azoospermia in medicine. If this situation is not caused by a physical blockage in the transport channels (Non-obstructive Azoospermia) and a distinct, clinical-level varicocele is detected on the patient’s examination, current medical approaches view surgical treatment as an extremely valuable ray of hope. In past years, these patients were directly directed to the procedure of searching for sperm surgically from inside the testicle (micro-TESE). However, today we know that the heat and toxic pressure created by severe varicocele may have temporarily stopped the sperm production factory in the testicle completely. Clinical studies have shown that after varicocele surgery in this group of azoospermic patients, natural live sperm may appear in the semen over the months in between twenty and seventy percent of patients. Even if sperm does not appear in the semen, since intratesticular blood flow and oxygenation improve, the chance of finding healthy cells in the subsequent sperm retrieval operation increases dramatically.

On the other hand, in patients whose sperm values are completely normal but whose lives are turned into misery due to unbearable testicular pain, the decision for varicocele surgery must be weighed much more carefully. Varicocele is not the only cause of testicular pain; hidden inguinal hernias, prostate or urinary tract infections, referred pain from kidney stones, or musculoskeletal system problems may also create similar pain. If the pain has the typical character of varicocele pain, increasing toward the end of the day and creating a feeling of heaviness and fullness in the testicle, and if all other disease possibilities have been excluded with detailed tests, surgery can be planned. In microsurgical operations performed only for pain management, the chance of complete resolution or major relief of the patient’s existing pain is approximately between fifty and seventy percent, and patients must definitely be informed about this level of expectation before surgery.

#col-2068773954 > .col-inner {
 margin: 0px 0px -45px
