This procedure aims to preserve kidney functions and prevent the development of hydronephrosis by restoring urine flow.

The symptoms and surgical indications of ureteropelvic junction obstruction generally present with clinical findings such as flank pain, urinary tract infections, and kidney enlargement. The narrowing confirmed by imaging methods requires surgical intervention if there is a risk of functional loss.

Laparoscopic and robotic pyeloplasty techniques are among the minimally invasive approaches and attract attention with their high success rates. These methods offer the advantages of less pain, shorter hospital stay, and rapid recovery, providing an alternative to classical open surgery.

The postoperative recovery process and follow-up protocol vary depending on the patient’s general condition and the surgical technique applied. With regular check-ups, imaging tests, and symptom follow-up, kidney functions are evaluated and possible complications are detected at an early stage.

Things You Should Know

Information

Purpose of the Surgery

To eliminate the urine accumulated in the kidney (hydronephrosis), reduce pain, lower the risk of infection, and prevent loss of kidney function.

In Which Situations Is It Applied?

It is preferred in the presence of advanced hydronephrosis, recurrent kidney infections, kidney stone formation, decrease in kidney function, and severe flank pain.

Causes

Congenital developmental anomalies, vascular compression (accessory renal artery), past infections, stone disease, or postoperative scar tissue.

Symptoms

Flank pain, abdominal pain, urinary tract infection, nausea-vomiting, irritability and failure to gain weight in children; in some cases it may be asymptomatic.

Diagnostic Methods

Ultrasonography, diuretic renal scintigraphy (MAG-3 or DTPA), computed tomography (CT), magnetic resonance urography (MRU), and intravenous pyelography.

Surgical Techniques

Open pyeloplasty, laparoscopic pyeloplasty, and robotic surgery. id="What_kind_of_problem_does_ureteropelvic_junction_obstruction_cause_in_our_body">What kind of problem does ureteropelvic junction obstruction cause in our body?

Our body has a tremendous balance and a flawlessly functioning plumbing system. Our kidneys are responsible for filtering our blood hundreds of times every day and removing harmful wastes and excess water from the body in the form of urine. Inside each kidney, there are more than a million tiny filtering units called nephrons. The urine droplets filtered from these units collect in a wide reservoir located in the middle part of the kidney and called the “renal pelvis.” This fluid accumulating in the pelvis is carried to the bladder through a thin, long, and flexible tube called the “ureter.”

The descent of urine through this tube is not a passive process driven by gravity. The urinary canal contracts regularly and rhythmically from top to bottom, just as our esophagus pushes swallowed bites toward the stomach. These milking movements are called peristalsis. When ureteropelvic junction obstruction is involved, there is a disruption in the muscle structure that initiates and maintains these rhythmic contractions. Most of the time, there is no physical stone or mass obstructing the urinary tract. Instead, the smooth muscle cells in that region are underdeveloped, or the muscle fibers have been replaced by hard, inflexible connective tissue (fibrosis). This non-contracting and non-stretching region prevents urine from being pumped downward. This condition is called functional obstruction.

As urine cannot descend, it begins to accumulate in the kidney pelvis. The pelvis expands like an inflated balloon in order to accommodate the increasing fluid volume. In medical terminology, this condition is called hydronephrosis. If this condition continues for a long time, the high hydrostatic pressure created by the urine trapped inside begins to press from within on the delicate fleshy tissue (parenchyma) of the kidney that filters the blood. The blood supply of the kidney tissue under high pressure becomes impaired, the cells cannot receive enough oxygen, and over time the tissue thins and completely loses its function.

We can divide the factors causing this narrowing into two groups according to their structural characteristics. The causes are generally as follows:

Intrinsic causes

Extrinsic causes

At the forefront of intrinsic causes is the segment that has lost its ability to contract (adynamic segment), which we mentioned just now. Sometimes small membrane-like congenital septa or polyps within the canal may narrow the passage path of urine. Extrinsic causes, on the other hand, work through a very different mechanism. The vascular network in our body may sometimes deviate from the standard anatomical structure. An extra artery carrying blood to the lower part of the kidney may pass right over this junction point and press on the urinary canal like a hose. This physical pressure coming from outside stops urine flow just like an intrinsic narrowing and causes the same harmful effects.

In whom is ureteropelvic junction obstruction seen more frequently, and is there a genetic transmission?

This disorder is one of the congenital urinary system anomalies encountered quite frequently worldwide. According to statistics, this problem is encountered in approximately one out of every one thousand five hundred live births. In fact, in large-scale ultrasound screenings conducted in some societies, it has been observed that this rate is much higher, approaching one in every five hundred births.

There is a significant difference between the sexes in terms of incidence. The likelihood of this narrowing occurring in boys is approximately twice as high as in girls. In addition, which side of the body the disease affects is not entirely coincidental. In the vast majority of cases, on average in a range of sixty to seventy percent, the obstruction occurs in the left kidney. This is thought to arise from anatomical placement differences of the left and right kidneys during embryological development in the womb. Although the disease usually affects one kidney, in a small proportion of patients the same narrowing picture may be seen in both kidneys.

As for the genetic aspect, in the majority of patients there is no history of such a disease in the family before. The event is entirely an incidental disruption occurring during the individual’s development in the womb. However, the medical world cannot say that the disease is completely independent of genetics. There are cases in some families where this problem is passed down from generation to generation and the same condition is seen in more than one sibling. This points to a multifactorial inheritance model in which multiple genes and environmental factors come together and trigger the process.

Although this disease is usually congenital, it does not always appear during infancy. Sometimes the narrowing is very mild and remains silent for years without causing any problem. When the person reaches their twenties or thirties, it may suddenly begin to produce symptoms due to changes in the kidney’s working pace or entirely different reasons. In addition, there are also narrowings that develop later in adulthood. Trauma to the kidney region caused by severe traffic accidents or falls, scar tissue left in the area by difficult surgeries previously performed due to kidney stones, or chronic severe kidney infections such as tuberculosis may cause the urinary canal, which was initially completely healthy, to narrow over the years.

Contact us now to get detailed information about treatments and to make an appointment!

 Contact via WhatsApp

What are the symptoms of ureteropelvic junction obstruction in infants and adults?

The way the disease reflects outward varies across a very wide spectrum depending on the age group of the patient, the degree of the obstruction, and whether there are other complications in the kidney. Thanks to today’s advanced obstetric practices, the vast majority of these patients are now diagnosed before they are even born, during routine ultrasound checks in the womb. On the ultrasound screen, it is detected that one of the baby’s kidneys is wider than normal. When these babies are born, they generally show no outward sign of disease.

However, the picture is different in infants and young children in whom it cannot be detected in the womb or whose obstruction is very severe. The main symptoms seen in children are as follows:

Fever

Vomiting

Loss of appetite

Irritability

Abdominal swelling

If we look at these symptoms in more detail, fever and irritability are generally the result of infections developing due to urine pooling in the kidney. Urine is normally a sterile fluid, but any place where flow stops and fluid accumulates becomes a perfect environment for bacteria to grow. Bacterial growth in the urine accumulated in the kidney causes severe febrile kidney infections (pyelonephritis). Since babies cannot express their pain, they show this condition through signs of restlessness such as constant crying, pulling their legs toward their abdomen, and refusing breast milk or formula. A severely enlarged kidney may reveal itself in the baby’s abdomen as a mass (swelling) that can be noticed even from the outside or felt by hand during the doctor’s examination.

In adults and adolescents, however, the situation is somewhat different. The main symptoms seen in this group are as follows:

Flank pain

Bloody urine

Kidney stones

High blood pressure

The most well-known of these symptoms in adults is a very characteristic pain pattern called “Dietl crisis” in the medical literature. This pain is not constant; it appears from time to time, intensifies, and then disappears on its own. It is especially closely related to fluid consumption. When a person drinks a large amount of water in a short time, consumes beverages such as tea or coffee that accelerate urine production, or drinks alcohol with diuretic properties, the kidney suddenly produces a very high amount of urine. This intense urine produced cannot pass through that narrowed canal and suddenly swells the renal pelvis. The sudden stretching of the outer capsule surrounding the kidney causes extremely severe pain radiating from the back toward the groin. As the person rests and the urine gradually filters through the canal, the pain also subsides and passes.

In adult patients, the prolonged retention of urine in the kidney (stasis) lays the groundwork for the minerals in the urine to precipitate and crystallize, ultimately resulting in kidney stone formation. Kidney stone complaints are also seen in approximately one-fifth of adults with narrowing. Sometimes patients may present to the doctor with visible bleeding in their urine (hematuria) after heavy exercise or after a mild blow to the kidney region. The kidney tissue compressed under high pressure for many years begins to secrete some hormones to protect itself, and this may lead to persistent and difficult-to-control high blood pressure in the patient.

How is ureteropelvic junction obstruction diagnosed in today’s medical technology?

The diagnostic process is not limited to seeing only how much the kidney has enlarged anatomically. What is truly important is to measure how much that enlargement has impaired the kidney’s filtering and drainage function. For this purpose, highly advanced technological devices serving different purposes are utilized.

The main imaging tests used are as follows:

Ultrasonography

Kidney scintigraphy

Computed tomography

Magnetic resonance imaging

Evaluation always begins with ultrasonography. Since the ultrasound device works with sound waves, it contains no radiation, causes absolutely no harm to the patient, is completely painless, and can be safely applied to all age groups. On ultrasound, the overall size of the kidney, the anterior-posterior width of the pelvis, how much the channels inside the kidney are swollen, and most importantly, the thickness of the fleshy kidney tissue are measured millimetrically. The fact that only the kidney is enlarged and no enlargement is seen in the urinary canal descending below is the strongest evidence that the problem is exactly at that junction point. These data obtained from ultrasound are numbered from one to four with a standard grading system accepted worldwide, and the severity of the disease is determined.

However, ultrasound only gives us an instant snapshot of the kidney; it does not show how the system works or how quickly urine can pass through that narrowed area. This is where kidney scintigraphy comes into play. Scintigraphy is a very special and dynamic test accepted as the gold standard for understanding whether the narrowing is truly harming the kidney.

During scintigraphy, the patient is given, intravenously, a very low-dose and safe radioactive substance that is filtered and excreted very quickly by the kidneys. The patient is placed under a special camera, and the arrival of this substance to the kidney with blood flow, its uptake by the kidney tissue, and its excretion through the urinary tract are monitored minute by minute. In the later minutes of the test, a diuretic drug is also injected into the patient. Our aim is to force the kidney to work at high capacity and see how that narrowed area will respond to the increased fluid load.

As a result of scintigraphy, the computer gives us two very critical numerical values. The first is the half-time. It refers to the time required for half of the drug accumulated in the kidney to pass through that narrowed canal and leave. If the pathways are open, this period is under ten minutes. If it takes longer than twenty minutes, it is mathematically proven that there is a serious and clinically significant obstruction in that area. The second important value is differential function. This value shows how the two kidneys share the total workload. In a healthy person, the kidneys share the work equally. If the workload share of the affected kidney falls below forty percent, this is a very clear alarm bell showing that the obstruction has now begun to strain the kidney and reduce its capacity.

Computed tomography and magnetic resonance (MR) imaging methods, on the other hand, usually come into play in patients whose condition is more complex. Especially in adults, these methods are used to map in high resolution whether there is an extra vessel pressing on the narrowing from outside and possibly constituting the real source of the problem, and the location and size of stones inside the kidney. In children and young people, MR imaging, which is safer, is preferred instead of tomography in order to prevent radiation exposure.

Contact us now to get detailed information about treatments and to make an appointment!

 Contact via WhatsApp

Is surgery necessary for every diagnosed case of ureteropelvic junction obstruction?

The presence of the words narrowing or obstruction in the name of the disease does not mean that every diagnosed patient will urgently be placed on the operating table. The basic philosophy of modern medicine is not to perform any unnecessary intervention on the patient, but also not to lose time in applying the correct treatment when there is a risk of permanent damage to the kidney.

A very patient course is followed especially in kidney enlargements detected in the womb or in the first months of life. A very large proportion of the narrowings detected in this period may return completely to normal within the first two years as the child grows, the tissues mature, and the urinary channels gradually stretch on their own. For this reason, babies are not operated on immediately; instead, they are enrolled in a very close follow-up program with ultrasound and scintigraphy at frequent intervals. If kidney enlargement has stopped, filtering capacity is preserved, and the baby has no recurrent infections, doctors prefer observation only, allowing nature to heal itself.

However, the observation process does not last forever, and things do not go smoothly in every patient. In some cases, the medical data begin to deteriorate, and surgical intervention is no longer an option but a necessity.

The conditions that definitely require surgery are as follows:

Loss of function

Persistent pain

Recurrent infections

Stone formation

Parenchymal thinning

Let us explain these medical necessities in a little more detail. The most important criterion is the filtering capacity of the kidney. If, on scintigraphy, the working percentage of the problematic kidney has fallen below forty percent, or if a decline of more than ten percent is observed in the kidney’s performance during sequential follow-ups, waiting may now lead to the complete loss of the kidney. Apart from this, if an adult patient has severe flank pain that appears with fluid intake and ruins daily quality of life and work life, a decision for surgery is made.

If the patient is continually experiencing febrile kidney infection attacks due to urine pooling in the kidney, surgery becomes mandatory because this rapidly ages the kidney tissue and carries the risk of bacteria entering the bloodstream. If stones large enough to require surgery have formed inside the kidney due to impaired urine flow, both the stones are cleared and the narrowing in the canal is corrected in a single operation. If ultrasound shows that the fleshy tissue of the kidney has begun to thin progressively under the increasing pressure, the narrowing must be opened immediately to save the kidney. In addition, although very rarely, some people are born with a single kidney, and if the narrowing is in this single kidney, timely intervention is of vital importance to prevent the patient from ending up on dialysis.

What are the modern surgical methods used in the treatment of ureteropelvic junction obstruction?

Once the decision for surgery is made, the aim of treatment is extremely clear: to completely eliminate that problematic, non-contracting, narrowed region, to suture the healthy tissues of the kidney and the healthy tissues of the urinary canal back together so as to form a wide opening, and to permanently remove the obstruction in front of urine flow. Today, various methods representing the most advanced point of technology are used to perform this procedure.

The main surgical techniques used are as follows:

Open surgery

Laparoscopic surgery

Robotic surgery

Endoscopic interventions

Among these methods, the procedure called pyeloplasty is based on cutting out the obstructed area and suturing the healthy ends together. While open surgery was the only option in previous years, today it is generally preferred only in very small babies, in situations where it is more suitable for their tiny bodies and gives quick results. The kidney is reached through an incision made on the side of the body, and the suturing procedures are performed directly with the surgeon’s hands.

Today, however, the standard in the medical world is minimally invasive, that is, closed surgical methods that cause the least harm to the body. Laparoscopic surgery is the first step of this revolution. Thin, long instruments and a high-resolution camera are placed inside through only a few millimetric small holes opened in the patient’s abdomen or back region. The surgeon performs the operation by watching a television screen and using long rod-shaped instruments. Since there are no large incisions in the patient’s body, postoperative pain is much less and the recovery process is faster.

However, when the work involves extremely delicate craftsmanship such as re-suturing the urinary canal to the kidney with millimetric stitches, robotic surgical systems come into play. Robotic surgery is an extraordinary technology that carries the limits of laparoscopy much further.

In the robotic system, the operation is not performed by a robot on its own; the surgeon sits at a special console right next to the patient and controls the robot’s arms with instruments at their fingertips. The camera placed inside provides the surgeon with a three-dimensional, very high-resolution, and depth-perceiving excellent field of view. The tiny instruments at the ends of the robot’s arms imitate the movements of the human wrist exactly and can even rotate fully around their own axis. Thus, even in narrow areas where the human hand would have difficulty reaching or turning, sutures can be placed with perfect precision and without the slightest tremor. Thanks to robotic surgery, operation times are significantly shortened, the quality of sutures increases, and patients can be discharged walking from the hospital just one or two days after surgery.

If the only cause of the narrowing is not a structural defect of the canal but an extra vessel pressing on the canal (extrinsic narrowing), a much more special method may be applied. In this case, the canal is never cut; the urinary canal is suspended like a bridge over that compressing vessel to a higher and safer point of the renal pelvis. Since the integrity of the canal is not disrupted at all, the risk of urine leakage drops to zero.

What is the recovery process and stent management like after ureteropelvic junction obstruction surgery?

Just as the sutures placed on the operating table must be flawless, the fact that these sutures heal and fuse without problems is also critically important for the success of the treatment. This pathway from the kidney to the bladder, newly sutured, needs to be supported from within during the healing process. Just as we immobilize and support a broken bone with a cast, a special support device that remains inside the body is used to protect the internal sutures.

Minutes before the end of the operation, an extremely thin, flexible, silicone tube starting from the kidney, extending along the entire canal, and ending in the bladder is placed. This is called a “Double-J Stent.” The function of this stent is to act like a scaffold around the sutured area so that the tissues heal without narrowing and to guide the urine directly into the bladder without leaking from the suture lines. The stent is not something visible from outside the body; it is completely internal.

The length of time the stent remains inside generally varies between two weeks and six weeks depending on the patient’s healing speed and the surgeon’s preference. During this time, patients can continue their daily lives. The internal stent may sometimes cause temporary complaints such as a mild burning sensation while urinating, an urge to urinate frequently, or very slight pink discoloration in the urine with movement. However, these are completely normal and end immediately when the stent is removed. Removal of the stent does not require another surgery. It is painlessly removed in outpatient clinic conditions within just a few minutes by entering through the urinary opening with an endoscopic instrument (cystoscopy).

Immediately after the operation, during the first few days in the hospital, a small drain tube is placed in the abdominal wall to drain any leakage that may come from the surgical area. This drain is removed once it is confirmed that no fluid is coming. In addition, patients generally get up and start walking the day after surgery and return to their normal diet within a very short time.

To be able to say that full recovery has been achieved, regular follow-up after surgery is essential. Seeing, in ultrasound controls performed months after discharge, that the swelling in the kidney has significantly decreased, the fleshy kidney tissue has recovered, and that in scintigraphy tests the urine drainage time has returned to normal limits is the confirmation that the treatment has fully succeeded.

What risks does untreated ureteropelvic junction obstruction carry, and does it recur?

Thanks to advancing surgical technologies, success rates are extremely high, and in the vast majority of cases patients achieve a definitive cure with a single operation that will last throughout their lives. However, because of the complex structure of the human body and the fact that healing mechanisms vary from person to person, there is always the possibility, although very rare, of encountering some undesirable situations:

In the early period, millimetric urine leaks may be seen from between the sutures placed. Thanks to the stent placed inside, the body repairs and closes these small leaks on its own within a short time.

The truly concerning situation is that months or years after the surgery, the area narrows again, in other words, the disease recurs. While healing tissues, our body has a tendency to leave scar tissue. If the wound healing on the suture line inside occurs by producing an excessive amount of hard connective tissue (fibrosis), or if there is a problem in the blood supply to that area, the newly created wide canal may gradually shrink and narrow again. Although this possibility is low, if it occurs, it may be necessary to enter from the inside with a closed method and cut that scar with a laser, or, much more rarely, to perform a second open/robotic repair surgery.

More dangerous than all this, however, is the failure to treat a diagnosed narrowing for which surgery has been recommended due to fear or neglect. As long as the narrowing is not opened, the destructive pressure inside the kidney continues to increase day by day. The kidney tissue, which tries to remain strong at first, cannot withstand this silent torture lasting for months and years. Kidney cells whose blood circulation is impaired die one by one, and the fleshy tissue melts away until it becomes as thin as a membrane. The kidney turns into a lifeless sac of water filled only with fluid and incapable of producing urine. This means that the functions of that kidney are irreversibly, completely, and forever lost.

#col-25192752 > .col-inner {
 margin: 0px 0px -45px
