This picture is characterized by symptoms such as frequent urination, urinary incontinence, straining during urination, and incomplete emptying.

Frequent urination and urinary incontinence are associated with uncontrolled contractions of the bladder muscles or pelvic floor weakness. Such symptoms reduce quality of life and are observed more frequently especially in the presence of advanced age, a history of childbirth, or neurological disease.

Straining during urination and intermittent urination generally develop due to underlying mechanical causes such as urinary tract obstructions or prostate enlargement. In men, benign prostatic hyperplasia and in women, urethral stricture are among the important factors.

Neurogenic bladder and voiding disorders originating from the nervous system occur due to loss of bladder control related to brain or spinal cord damage. Diabetes, multiple sclerosis, and spinal cord injuries may cause this condition and require multidisciplinary treatment.

Things You Should Know

Information

Definition

Voiding disorders (micturition disorders) refer to functional or structural problems that arise during the storage and/or emptying of urine. id="What_Is_Urethral_Stricture_Which_Is_Commonly_Encountered_Among_Voiding_Disorders">What Is Urethral Stricture, Which Is Commonly Encountered Among Voiding Disorders?

The urinary canal, medically called the urethra, is an anatomical structure in the form of a flexible tube that carries urine from the bladder to the outside of the body. Urethral stricture is the narrowing of the inner lumen of this carrying canal as a result of the development and hardening of scar tissue in the spongy tissue surrounding the canal for various reasons. To explain this with an example from daily life, it can be thought of like the inner surface of a flexible and smooth water hose becoming narrowed over time due to limescale buildup, making it difficult for the water flowing through it.

The causes leading to narrowing of the canal vary greatly depending on the region where the stricture occurs. Narrowings seen in the parts of the urinary canal closer to the outside generally develop as a result of cellular trauma arising after severe past infections, irritation caused by previously inserted urinary catheters, or certain medical procedures such as some prostate surgeries performed with closed methods. In addition, hard impacts to the perineal area from falling off a bicycle or while riding a horse also trigger scar tissue formation by crushing the spongy tissue in this region. Strictures in the deeper regions of the urinary canal, toward the inner parts of the body, are mostly seen after much more severe trauma such as pelvic fractures resulting from traffic accidents or after complex surgical interventions.

What Are the Symptoms of Urethral Stricture and How Is the Diagnosis of Voiding Disorders Made?

Due to the narrowing of the canal, the bladder has to generate much more pressure than normal and strain in order to expel the urine. Over time, this causes the bladder muscle to tire, lose its elasticity, and leave a constant amount of urine inside. The urine remaining inside creates an ideal environment for bacteria to multiply, paving the way for recurrent infections.

The most frequently observed complaints in patients are as follows:

A marked decrease in urinary stream force

Difficulty starting urination

Forked or scattered urine flow

The feeling of not being able to empty the bladder completely after urination

Continued dribbling after leaving the toilet

The need to go to the toilet frequently

The first step taken in the diagnostic process is the uroflowmetry test, in which the speed and shape of the patient’s urine flow are measured electronically. Immediately after this test, the amount of urine remaining in the bladder after urination is measured millimetrically with the help of an ultrasound device. However, in order to make the definitive diagnosis and to be able to see like a map where the stricture begins, how long it is, and how much the canal has narrowed, films are used that are taken by giving a special contrast fluid into the urinary canal. Thanks to these films, it becomes possible to make a surgical plan. In more complex cases when deemed necessary, the stricture area is examined live and the character of the tissue is observed by entering directly into the urinary canal with the help of thin flexible instruments with a tiny camera at the tip.

Which Surgical Methods for Voiding Disorders Are Used in the Treatment of Urethral Stricture?

The success of surgical interventions in this field depends on the sensitivity to preserve the extremely complex vascular and nerve network around the urinary canal. The nutrition of the tissues in this region is provided by a dual-direction blood flow system. During surgical repair, the preservation of these vessels and especially the nerve branches managing sexual functions is extremely critical both for the tissue to heal healthily and for the patient’s quality of life not to decline.

The main surgical treatment methods applied are as follows:

Urethral dilation procedure

Closed incision of the canal with laser or knife

Removal of the problematic tissue and end-to-end reconnection of the canal

Widening of the canal with a graft taken from inside the mouth

The urethral dilation procedure is a method applied only in very short and superficial strictures and is based on mechanically stretching the canal with the help of special tubes. However, since the actual hardening problem in the tissue is not eliminated, this dilation usually provides temporary relief and the likelihood of the stricture recurring is very high.

In the closed canal incision method, the urinary canal is entered with a small camera and the scar tissue at the point of the stricture is carefully incised using a cold knife or laser beams. The procedure time is short and it is easy to apply; however, if deep incisions are made, there is a risk of opening unwanted passageways between different tissues:

End-to-end anastomosis surgeries are the methods that usually provide the most permanent and successful results in strictures shorter than two centimeters. In this method, the diseased tissue piece that has lost its elasticity and causes the narrowing is completely cut out and removed. The two remaining healthy ends of the canal are joined together with extremely fine sutures.

The graft method is used in longer and more complex strictures that would create tension if reconnected end-to-end. In this technique, the canal needs to be supported with a completely new tissue. Most often, a mucosal tissue taken from the inner cheek of the patient, which is resistant to moisture, is stitched to the narrowed urinary canal like a patch to widen the diameter of the canal. Depending on the difficulty of the case, this repair may be completed in a single operation or planned in two separate sessions lasting for months in order to guarantee healthy healing of the tissue.

How Does Overactive Bladder Develop Within the Scope of Voiding Disorders?

Overactive bladder is the condition in which the bladder suddenly begins to contract completely inappropriately and beyond the person’s control before reaching full physical capacity. Under normal conditions, a healthy bladder gradually stores the urine coming from the kidneys at a very low pressure. During this filling phase, the bladder muscle is expected to remain completely relaxed. However, when this functioning is impaired, the receptors in the wall of the bladder that govern contraction begin to produce false signals. These receptors constantly send urgent warnings to the brain as if the bladder were completely full and about to overflow. When the brain receives these false warnings, it gives the bladder muscle the command to contract, and the person suddenly feels the need to run to the toilet. This can be likened to an overly sensitive fire alarm that starts ringing constantly by mistake and cannot be silenced.

What Are the Diagnostic Process and Symptoms of Overactive Bladder Within the Scope of Voiding Disorders?

The complaints caused by this condition can make it almost impossible for a person to board a bus, go to the cinema, or sleep uninterruptedly at night. Although the severity of the condition varies from person to person, its effect on quality of life is extremely disruptive.

The main symptoms through which the disease manifests itself are as follows:

A sudden and absolutely unpostponable severe need to use the toilet

Going to the toilet much more often during the day than is considered normal

Waking up repeatedly from sleep at night with the sensation of needing to urinate

Dribbling or completely losing urine before reaching the toilet

In the diagnostic process, not only what the patient tells but also objective data documenting their daily routines are of great importance. For this reason, patients are asked to keep a detailed voiding diary for at least three days during the diagnostic stage. In this diary, the amount of water the person drinks during the day, the times they go to the toilet, how much urine they produce, and whether leakage occurs are recorded. These records serve as a very valuable map for understanding at what degree of bladder fullness the muscle is giving false signals. Of course, all other possibilities that may cause similar complaints, such as a simple urinary tract infection or the presence of a small stone in the bladder, are also ruled out in detail through simple laboratory tests and imaging devices.

Contact us now to get detailed information about treatments and to make an appointment!

 Contact via WhatsApp

Which Treatment Steps Are Applied for Overactive Bladder?

The management of overactive bladder syndrome always begins with the most natural methods, without side effects, and with the least intervention to the body’s structure. If these methods are not sufficient, it progresses gradually toward more advanced and complex medical steps.

The methods gradually applied during the treatment process are as follows:

Rearranging the daily amount of fluid consumption

Removing foods that irritate the bladder muscle from the diet

Special training aimed at increasing bladder capacity

Exercise programs that strengthen the pelvic floor muscles

Drug treatments that stop involuntary contractions

Botox injections applied to the bladder wall

Bladder pacemaker application that regulates nerve stimuli

First-line treatments focus entirely on the person’s own lifestyle changes. The daily amount of water consumed is distributed evenly over time. In addition, tea, coffee, carbonated beverages, and overly spicy foods are removed from the diet because they irritate the inner surface of the bladder and trigger false alarms. In the process called bladder training, when the person feels a sudden urge, instead of immediately running to the toilet, they remain in place, squeeze their pelvic muscles, and consciously learn to suppress this urgent contraction feeling. In this way, the intervals between toilet visits are gradually lengthened over time.

In the second stage, if behavioral changes alone are not sufficient, special oral medications come into play. The main purpose of these drugs is to stop involuntary contractions by blocking those receptors in the bladder muscle that give false alarms. While some drug groups stop the contraction signal, newer-generation drug groups increase the bladder’s relaxation capacity, allowing more urine to be stored inside at a lower pressure.

In the third and more advanced stages, cases resistant to medication are focused on. By entering the bladder with camera systems, botox injections are applied to dozens of different points in the muscle wall. This procedure temporarily paralyzes the overreactive muscle fibers for months and prevents the bladder from contracting suddenly. In another advanced method, the bladder pacemaker application, mild electrical currents are sent through a thin wire placed on the nerves in the sacrum. These currents reorganize the disrupted communication network between the brain and the bladder and allow the system to function correctly.

What Are the Types of Urinary Incontinence Within the Scope of Voiding Disorders?

Although urinary incontinence is very common in society, it is generally hidden, not often expressed due to embarrassment, but it is a condition that practically imprisons a person’s social life. Although many people think that this condition is only a natural result of aging, in fact there are very different mechanisms and anatomical problems underlying it:

The basic types of urinary incontinence are as follows:

Stress incontinence related to increased intra-abdominal pressure

Urgency incontinence related to a sudden urge to urinate

Mixed incontinence in which both conditions are seen at the same time

Stress incontinence occurs as a result of the muscles and connective tissues supporting the bladder and urinary canal from below losing their strength over time. When the person laughs, coughs violently, sneezes, lifts a heavy object, or climbs stairs, the pressure in the abdominal cavity suddenly increases. Since the supportive tissue below is weak, the urinary canal cannot resist this pressure, the valve system loosens, and the person involuntarily leaks urine. This condition is especially common in women who have had multiple difficult vaginal deliveries or after menopause when the tissues lose their elasticity.

Urgency incontinence is the extreme point of the overactive bladder condition explained in detail earlier. The bladder contracts so suddenly and so forcefully that even if the valve system is functioning normally, it cannot withstand this enormous pressure and the person leaks urine before they can reach the toilet. In mixed type, both of these mechanisms are present in the same patient; that is, the patient leaks both when coughing and when suddenly feeling the urge to urinate and being unable to get to the toilet in time.

Which Surgical Solutions Are Applied for the Problem of Urinary Incontinence?

The permanent and most effective solution for stress urinary incontinence is surgical interventions aimed at restoring anatomical support. The basic logic of the methods developed for this purpose today, with extremely high success rates, is to support the urinary canal, which has weakened and sagged downward, from below like a hammock with a synthetic mesh-like patch.

The most frequently applied hammock or sling surgeries are as follows:

Sling surgeries suspended to the abdominal wall

Sling surgeries passed through the groin folds

In sling surgeries suspended to the abdominal wall, the ends of the synthetic patch passed just under the urinary canal are placed by creating tunnels behind the pubic bone upward toward the abdominal muscles. In this way, whenever intra-abdominal pressure increases, this synthetic hammock slightly compresses the urinary canal upward and prevents urine from leaking out. However, since there is a risk of the needles accidentally perforating the bladder wall during this procedure, the inside of the bladder must be checked with camera systems during surgery.

In the method passed through the groin folds, the ends of the synthetic patch are passed horizontally through the anatomical spaces in the region where the thigh bones join without directing them into the abdominal cavity at all. Thanks to this horizontal passage, the bladder wall is never approached, and therefore the risk of bladder injury is almost completely eliminated. This method is considered anatomically much safer and produces excellent results in the long-term quality of life of patients. During the first six-week healing period after surgery, it is an indispensable rule that the patient absolutely must not lift heavy objects, should be protected from severe coughing crises, and should take care not to become constipated, so that the synthetic patch can fully integrate with the body’s tissues and the surgery can remain permanent for a lifetime.

How Is the Relationship Between Neurogenic Bladder and the Nervous System Explained?

The act of the bladder expanding and stretching while filling with urine and then contracting to expel the urine completely when it is time to void is actually made possible by a tremendous exchange of electrical signals flowing uninterruptedly between the brain, spinal cord, and urinary tract. We can liken this to a massive computer network; the brain, as the main computer, constantly sends commands to the bladder, the terminal unit, through the spinal cord, the cables, and receives instant status reports from it. Neurogenic bladder is the condition in which the harmony between the bladder and the sphincter muscle holding the urine is completely lost as a result of damage occurring at any point in this flawless neural communication network.

The main diseases that interrupt neural communication and cause neurogenic bladder are as follows:

Brain hemorrhages or stroke conditions

Multiple Sclerosis disease

Parkinson’s disease

Spinal cord paralysis due to traffic accidents or falls

Congenital spina bifida conditions in which the spine does not close

One of the most dangerous conditions resulting from these diseases, which can even lead to organ loss, is the complete opposition and stubborn working of the bladder muscle and the sphincter muscle that holds the urine. In a normal healthy voiding moment, while the bladder muscle contracts to push the urine out, the sphincter muscle serving as a gate should open fully and relax. However, when neural harmony is impaired, while the bladder contracts with all its force trying to expel the urine, the sphincter muscle, on the contrary, stubbornly contracts and locks the gate. This causes enormous pressure to build up inside the bladder. The urine that cannot come out returns backward under the effect of the high pressure, creating a reverse flow toward the kidneys, and over time permanently damages the delicate filtering cells of the kidneys, leading to kidney failure.

Contact us now to get detailed information about treatments and to make an appointment!

 Contact via WhatsApp

What Treatments Are Applied in Neurogenic Bladder Patients and Urodynamics?

In order to detect this hidden and extremely dangerous pressure, it is mandatory to perform a computerized and highly detailed pressure measurement test called urodynamics. This test focuses not only on how much fluid the bladder takes in but also on how much physical tension it creates in its wall while taking in that fluid. Here, in order to find that pure and dangerous force produced by the bladder muscle itself, a very clear mathematical equation is used:

P_{det} = P_{ves} – P_{abd}

The basic logic of this formula is as follows; the total pressure inside the bladder is not generated only by the contraction of the bladder muscle. When a person breathes, moves, or tightens the abdomen, the organs within the abdomen also put weight on the bladder. Therefore, subtracting this intra-abdominal weight from the total pressure and calculating only the actual dangerous force produced by the bladder muscle itself is essential for understanding the severity of the disease.

In the treatment of patients with neurogenic bladder, the first and most vital priority is to protect the kidneys by lowering that increased pressure at all costs.

The methods applied to protect the kidneys and ensure urine drainage are as follows:

Emptying the bladder at regular intervals by placing disposable thin tubes into it

Using high-dose oral medications that relax the bladder muscle

Applying botox injections to the closed urinary sphincter muscle

Surgical enlargement of the bladder using pieces of intestine

The self-catheterization method is a life-saving routine procedure for patients whose bladder cannot push out urine. During the day, the patient sets an alarm, inserts clean and lubricated thin catheters into the urinary canal by themselves, drains the urine accumulated in the bladder, and throws the catheter away. This simple but effective physical drainage prevents the accumulation of high-pressure urine inside the bladder. If despite medication the bladder wall has shrunk, thickened, and completely lost its elasticity, a patch taken from the patient’s own intestines is surgically stitched to the bladder. In this way, the volume of the bladder is increased, the internal pressure is permanently reduced, and the kidneys are secured.

What Is Pelvic Organ Prolapse and Why Does It Occur?

Pelvic organ prolapse is the condition in which the lower parts of the bladder, uterus, and intestines located in the pelvic cavity of women herniate downward and protrude through the vaginal canal due to gravity as a result of weakening of the connective tissues and muscles that hold them in place. You can think of the muscles forming the floor of the pelvis like a wide and tight trampoline. All internal organs sit safely on top of this trampoline. However, when the springs of the trampoline loosen, the weights on it begin to sag downward.

The risk factors that loosen the springs of the trampoline and cause prolapse are as follows:

Multiple and difficult vaginal deliveries

Connective tissue weakness associated with advanced age

Chronic increase in intra-abdominal pressure due to excessive weight

Continuous and severe coughing due to conditions such as asthma

Chronic constipation and prolonged severe straining

A special mapping system is used to be able to speak about the severity of the prolapse in a common standard in medical language. In this system, the hymenal region of the woman is accepted as the zero point, that is, the reference center. The millimetric distance of nine different anatomical points on the inner walls of the vagina to this reference center is measured. While the absence of prolapse is called Stage 0, the most advanced condition in which the uterus and bladder are completely turned inside out and protrude out of the vagina is classified as Stage 4.

Which Surgical Methods Are Applied in Pelvic Organ Prolapse?

The process of surgically restoring the prolapsed organs to their former positions is carried out through different methods meticulously selected according to the patient’s age, which organs are causing the prolapse, and whether the woman’s sexual life is active or not.

The main reconstructive surgeries applied in treatment are as follows:

Removal of the uterus through the vaginal route and repair of the hernias

Suspension of the organs upward with robotic surgical systems

Short sling suspension procedures performed without entering the peritoneum

Complete closure of the vaginal canal for elderly patients

In repairs performed through the vaginal route, there is no externally visible incision on the patient’s abdomen. The procedure is performed entirely through the vaginal cavity; the prolapsed uterus is removed, and the bladder protruding from the front and the bowel tissue herniating from the back are stitched and restored using their own supporting ligaments. In robotic surgery, high-resolution 3D cameras sent into the abdomen and robotic arms with much more flexibility than the human hand are used. The prolapsed vaginal vault or uterus is suspended with a very strong synthetic mesh to the strong bony structure at the lowest point of the spine with strong sutures. This suspension procedure is so strong that the risk of the disease recurring in the future is below even two percent. If the patient is of very advanced age, if receiving anesthesia carries major risks, and if she does not have sexual intercourse, the walls of the vagina are stitched over each other so that the space is completely closed. This is a very practical solution that takes quite a short time and immediately relieves the patient of the prolapse problem.

Why Do Urinary Tract Infections Recur Frequently?

Urinary tract infections mostly occur when bacteria naturally present in the person’s own digestive system and intestinal flora leak into the urinary canal from the outside environment, attach to the inner surface of the bladder, and multiply there, initiating inflammation in the tissue. The anatomical structure of women is very different from that of men. In women, the urinary canal is only a few centimeters long, and this short distance creates a very easy path for bacteria to leak from the outside into the safe environment of the bladder. In men, this canal is much longer, so it is physically more difficult for bacteria to reach the bladder.

When a simple bladder infection begins, the patient experiences severe burning during urination, stinging as if cut by a razor, and cloudy urine. With the appearance of leukocytes, which are defense cells, in a simple urine test, short-term antibiotic treatment is immediately initiated. However, if these bacteria attached to the bladder get the opportunity to climb further upward, following the urine-carrying tubes to the kidneys, the condition becomes much more dangerous. Kidney infections manifest with high fever, chills, and unbearable kidney pain in the flanks. In this case, a urine culture is performed to find which bacterium caused the infection, and the microbes are grown in the laboratory and tested one by one to determine which antibiotic defeats them. Afterwards, much longer and stronger treatments administered intravenously have to be applied.

What Are the Ways to Prevent Urinary Tract Infections?

If a person has more than two urinary tract infections within six months or more than three within one year, it indicates that a cycle of recurrent infection has been entered. In these patients, using boxes of antibiotics each time causes the bacteria to mutate over time, gain resistance to those antibiotics, and the drugs become completely ineffective. Therefore, the main goal is to build protective walls that can prevent the infection before it even begins.

The protective supplements and methods used to break the infection cycle are as follows:

Very low-dose antibiotic protection taken at bedtime for months

Single-dose prophylactic medications taken after intercourse

Vaccine pills that train the immune system against microbes

Herbal cranberry extracts and natural sugar supplements that make the bladder wall slippery

Local hormone creams that strengthen vaginal tissue during menopause

If most of the infections occur immediately after sexual intercourse, protection in the form of a single pill taken only after that activity prevents bacteria from multiplying at the first moment of entry. Vaccine-like oral pills given to strengthen the immune system enable the body to recognize that specific bacterium and eliminate it at the threshold. Especially cranberry extracts and special natural supplements called D-mannose make the inner surface of the bladder slippery like a Teflon pan. Bacteria cannot physically adhere to this slippery surface and are flushed out of the body with the urine flow. After menopause, however, the beneficial and protective bacteria in the vaginal area die due to the decrease in female hormones, which opens the way for harmful bacteria. Thanks to externally applied local hormone creams, that tissue comes alive again, beneficial bacteria return, and a natural acidic defense line is re-established against infections.

What Is Interstitial Cystitis (Painful Bladder) and Why Is It Difficult to Diagnose?

Interstitial cystitis is a mysterious and chronic inflammatory disease that behaves just like a constantly recurring urinary tract infection from the outside, but although there is not a single bacterium in the patient’s urine, it causes terrible pelvic pain that increases as the bladder fills and a constant urge to go to the toilet. The diagnosis of this disease is quite difficult because the complaints are exactly the same as simple infections, but despite using boxes of antibiotics for weeks, there is not the slightest improvement in the complaints. In the medical process, the diagnosis of this disease is made by the method of “exclusion”; that is, after possibilities such as stones, tumors, and microbes are searched for one by one and cannot be found, only this syndrome remains.

The inner surface of our bladder is covered with a waterproof layer that prevents the acidic substances in the urine from leaking into the tissues. In individuals with this disease, it is thought that this protective barrier has thinned regionally, been perforated, and damaged. Once its waterproof property is lost, the acidic chemicals in the urine directly contact the exposed nerve endings in the bladder wall, creating a searing wave of pain. To confirm the diagnosis, the patient’s bladder is distended and filled with a special fluid under anesthesia. During this distension, the appearance of tiny bleeding spots in the bladder wall observed with a camera or the detection of special ulcerative structures formed on the wall is considered definitive evidence that the protective layer has collapsed.

Which Foods Should Interstitial Cystitis Patients Avoid?

Because the protective barrier on the inner surface of the bladder is weakened, every fluid consumed is filtered and mixed into the urine, and when it comes into direct contact with this damaged surface, the acidic or irritating contents of some foods and beverages that would not harm a normal person increase the pain and burning to incredible levels when they touch the bladder wall of these patients, which is like an open wound.

The main foods that must definitely be removed from the diet are as follows:

Tea

Coffee

Carbonated and fizzy drinks

Alcohol

Excessively spicy and hot foods

Citrus fruits such as lemon and orange

Chocolate

Artificial sweeteners

Even when these foods and beverages are completely removed from the menu, a serious reduction in the patients’ complaints and pain attacks is observed.

Which Steps Are Followed in the Treatment of Interstitial Cystitis?

The basic aim of the treatment is to somehow repair that damaged protective barrier and silence the pain cries sent to the brain by the exposed nerve endings in that region.

The stepwise treatments applied to bring the disease under control are as follows:

Use of special oral pills aimed at repairing the damaged barrier

Soothing medications that suppress allergic and neural reactions

Administration of special liquid medication mixtures into the bladder through catheters

Botox injections applied into the bladder to cut nerve impulses

In cases where dietary changes alone are not sufficient, special molecule-containing oral pills are used that slowly try to repair the bladder wall over a treatment lasting months. However, it takes a very long time for these pills to show their full effect. Therefore, instead of waiting for the drugs to pass through the stomach and intestines and enter the bloodstream, a much faster and more effective route is preferred. With the help of a catheter, special restorative acids and fluids are administered directly into the bladder from the outside. These fluids coat the damaged walls of the bladder from the inside like a bandage or armor, preventing the acid from reaching the nerves. In addition, powerful anti-inflammatory agents added to this liquid mixture provide great relief to the patient by numbing the nerves in that region. In very resistant and stubbornly painful cases, tiny botox injections are directly applied to that area in order to temporarily block the pathways carrying the pain signal to the brain.

#section_1970970560 {
