This condition occurs as a result of the weakening of bladder control mechanisms and can be significantly brought under control with the appropriate approach.

Stress urinary incontinence treatment is managed with pelvic floor muscle exercises, weight control, and surgical interventions when necessary. This condition, which is especially common after childbirth or during menopause, can be significantly improved by strengthening the muscles, and the patient’s quality of life can be enhanced.

Urge urinary incontinence treatment methods are planned through bladder training and the use of anticholinergic medications. In this condition, which develops due to the overactivity of the bladder muscles, regular toilet habits and pharmacological support play an effective role in controlling the symptoms.

Advanced urinary incontinence treatment options include botulinum toxin injection, neuromodulation, and surgical procedures. These methods, applied in cases where no response is obtained from conservative treatments, may provide lasting improvement by aiming to reorganize bladder functions.

What You Need to Know

Information

Definition

Urinary incontinence is the condition in which a person is unable to hold urine involuntarily. id="How_Does_Our_Bodys_Urinary_Continence_Mechanism_Normally_Work">How Does Our Body’s Urinary Continence Mechanism Normally Work?

In order to fully understand the causes, mechanisms, and treatment methods of urinary incontinence, it is first necessary to take a close look at the magnificent mechanism of a healthy excretory system in the human body that functions like clockwork. The structure we call the lower urinary system basically has two main functions that complement each other. The first is to store urine, which is continuously filtered from the kidneys, in a safe, low-pressure environment; the second is to remove this accumulation from the body smoothly when a socially appropriate time and place are available.

The urinary bladder, namely the bladder, can be thought of as an intelligent balloon made of muscles with extremely high flexibility. While urine drips into this sac from the kidneys, the special muscle layer in the bladder wall (the detrusor muscle) slowly relaxes. Thanks to this relaxation, even if the amount of fluid inside increases, the pressure inside the bladder remains constant and low. In this way, urine does not flow back to the kidneys. During this “storage” phase, the valve mechanism called the sphincter, which surrounds the channel (urethra) that allows urine to exit the body, must remain tightly closed. This process is under the flawless control of the sympathetic branch of our autonomic nervous system.

When the person decides to go to the toilet and takes the appropriate position, the wonderful coordination called the “voiding” phase begins. The upper centers in the brain send signals to the control points in the spinal cord. While these signals transmit the command “you can contract now” to the bladder muscles, they simultaneously send the message “relax immediately” to the valve muscles that keep the urinary tract closed. Thus, the urinary bladder contracts strongly and pushes the fluid inside outward, the valves open without resistance, and a healthy, uninterrupted urination takes place. Especially in female anatomy, there is a structure called the “pelvic floor” so that this system can withstand gravity and intra-abdominal pressure. The pelvic floor is a strong network of muscles and connective tissue that covers the lower part of the pelvis from end to end and supports the bladder, uterus, and intestines from below like a sturdy “hammock” or “suspension bridge.” When this hammock-like support mechanism weakens, stretches, or tears for any reason, the delicate balance of the system is instantly disrupted and complaints of inability to hold urine begin.

What Types of Urinary Incontinence Are There and What Are Their Causes?

Not every case of urinary leakage occurs through the same mechanism. Finding out what the underlying main problem is the most critical turning point on the road to the correct treatment. When the complaints increase during the day, which physical activities trigger them, or what kind of sensations they come with play a key role in determining the type of problem. Medically, there are basically three different main categories.

The first and most common type is stress urinary incontinence. Here, the word “stress” does not refer to psychological tension, sadness, or anxiety in daily life; it refers entirely to the physical mechanical pressure on the abdominal area, in other words, anatomical stress. It is involuntary urinary leakage that occurs in situations that suddenly increase intra-abdominal pressure, such as coughing, severe sneezing, laughing, lifting a heavy shopping bag, climbing stairs, exercising, or suddenly standing up. It is generally seen in women, and the number one cause of this is the weakening of the pelvic floor muscles we mentioned earlier. Due to this weakness, when the pressure inside the abdomen increases, the urinary tract cannot remain fixed in place, sags downward (hypermobility), and urine leaks because it cannot fully perform its valve function.

The main risk factors that trigger stress urinary incontinence are as follows:

Obesity

Advanced age

Difficult vaginal deliveries

Chronic cough

Persistent constipation

Menopause

The second type is urge urinary incontinence. This picture is characterized by a very strong urge to urinate that comes on suddenly and is almost impossible to postpone or stop. Even if the person knows where the toilet is and gets to the door, they may still be unable to make it in time and leak urine even while trying to put the key in the lock. The main culprit behind this condition is the “Overactive Bladder” syndrome. The bladder muscle begins to contract involuntarily and forcefully, independently of the brain, even when there is still very little urine inside. Excessive sensitivity in nerve cells, the damaging effect of diabetes on the nerves, or age-related cellular changes can lead to this condition.

The third main category is overflow urinary incontinence. This condition is like the bladder becoming filled to the brim, reaching the final limits of its stretching capacity, but being unable to empty the fluid inside due to various reasons, resulting in urine leaking out drop by drop or as a constant seepage, like a glass overflowing. There is usually a mechanical obstruction or a neurological paralysis underlying this picture. In men, the blockage of the urinary tract by benign prostate enlargement or the complete loss of the bladder muscle’s ability to contract due to many years of uncontrolled diabetes (lazy bladder) are the most prominent causes. Even if these people go to the toilet, they can never empty their bladders completely and there is always a high amount of residual urine left inside.

Contact us now to get detailed information about treatments and to make an appointment!

 Contact via WhatsApp

What Diagnostic Methods Are Used When Presenting with a Complaint of Urinary Incontinence?

The most important stage in the process of solving the problem is to transform the patient’s subjective complaints into scientific, measurable, and objective medical data. Proper and lasting treatment planning is only possible with an accurate diagnosis. The diagnostic journey begins with a very detailed interview conducted in the atmosphere of a warm conversation, but in which medical details are examined meticulously.

The specialist asks in detail how long the urinary incontinence has been present, how many protective pads are changed on average per day, at what moments the leakage is triggered, and how many times the patient wakes up at night to go to the toilet. The patient’s existing diseases, previous surgeries, and medications used regularly are reviewed one by one. Because sometimes diuretic pills used in the treatment of high blood pressure, some muscle relaxants, or psychiatric medications alone may be the main cause of urinary incontinence. In the physical evaluation, especially in female patients, bladder prolapse, uterine prolapse, or thinning of the vaginal tissues due to menopause (atrophy) are examined. If necessary, the patient may be asked to strain or cough forcefully to confirm whether the leakage is visibly present.

The basic tests we frequently use during the diagnostic stage are as follows:

Detailed patient history

Physical examination

Urinalysis

Voiding diary

Pad tests

Ultrasonography

The voiding diary is an extremely simple yet highly valuable tool, considered the gold standard in making a diagnosis. The patient is usually asked to write down in a small notebook for three consecutive days what time and how much and what type of fluid they consume during the day, what time they go to the toilet, the amount of urine, and whether they experience leakage. This diary acts like a mirror regarding the bladder’s actual capacity and the person’s fluid consumption habits. Pad tests are used to measure the severity of the condition in grams; the pads worn for a certain period are weighed on sensitive scales and the amount of leaked urine is mathematically calculated. In addition, with a simple urinalysis, it must definitely be ruled out whether a hidden infection is mimicking urinary incontinence.

Why Is the Urodynamic Test So Important in the Diagnosis of Urinary Incontinence?

While many cases can be resolved with only a good history and simple tests, in patients who are more complex, who have previously tried treatment but failed, who have neurological diseases, or for whom a surgical operation is planned, the system must be examined much more deeply. This is where urodynamic evaluation comes into play. Urodynamics is a highly advanced set of functional tests that maps the fluid dynamics, pressure relationships, and contraction abilities of the lower urinary system, namely the bladder and urinary tract, with millimetric precision on a computer. This set of tests shows exactly where in the plumbing there is a blockage, where there is a leak, or where there is a pressure abnormality.

The first stage is generally a simple measurement called uroflowmetry. The patient urinates normally into a special toilet equipped with sensitive sensors. The computer graphically draws the urine flow rate per second, the strength of the flow, and whether it is interrupted. A weak and prolonged curve usually indicates a blockage in the urinary tract or that the bladder muscle is very weak.

In the cystometry phase, which is a more advanced step, very thin and soft catheters, as thick as spaghetti, are placed into the bladder through the urinary tract. Through these thin tubes, sterile fluid is slowly started to be filled into the bladder. During this, how much the bladder can stretch, how the pressure inside changes, and when and how intensely the patient first experiences the feeling of “I need to go to the toilet” are followed instantly on the monitor. Whether there are involuntary and uncontrolled contractions while the bladder is filling is directly observed. In the final stage, a pressure-flow study is performed; the patient urinates while the catheters are still inside, and at this time the bladder pressure and the urine خروج speed are measured simultaneously, mathematically proving the degree of obstruction.

What Are the Non-Surgical and Natural Methods in the Treatment of Urinary Incontinence?

The unchanging golden rule of modern medicine and the urological approach is always to start treatment first with natural methods that require the least intervention, do not harm the body, and are risk-free and without side effects. The success of this stage, which we call conservative treatments, depends entirely on the patient’s determination, motivation, and strict implementation of the recommended lifestyle changes. This stage is, in a way, the process of the patient learning to work in harmony with their own body again.

Planning fluid intake correctly is the basis of the matter. Taking around two liters of fluid per day is essential for healthy kidney function, but the timing and content of this fluid are very important. Some substances that directly irritate the bladder muscle, facilitate its stimulation, and trigger the sudden feeling of urgency must definitely be removed from the diet or minimized.

The main foods that stimulate the bladder and can increase urinary incontinence attacks are as follows:

Strong tea

Coffee

Carbonated drinks

Alcohol

Spicy foods

Artificial sweeteners

Weight control is like a hidden hero in solving this mechanical problem. Excess weight around the waist and abdomen combines with gravity to apply continuous pressure to the pelvic floor muscles 24/7. Even a five percent reduction in body weight can reduce urinary incontinence attacks to an incredible extent. Similarly, chronic constipation is also an integral part of the treatment because full and hardened intestines cause constant physical pressure on the bladder right in front of them, so ensuring bowel regularity by eating fibrous foods is an inseparable part of treatment.

On the physical side of these natural methods is pelvic floor muscle training, known as Kegel exercises. These exercises are especially effective in stress and mixed urinary incontinence. The aim is to strengthen and tighten that “hammock” structure that supports the bladder from below. To find the correct muscle group, patients first try to stop the urine flow briefly while in the toilet; the muscles that stop the urine are the pelvic floor muscles that need to be exercised. However, the exercises should not be done in the toilet, but while sitting or lying down during the day, with the bladder completely empty. Patients learn to strongly contract these muscles for five to ten seconds, then completely relax them for twice that length of time. When done regularly, a very marked improvement is seen within months, almost as if the muscles have received a rejuvenation treatment. For those with urge incontinence, “bladder training” is applied. In this technique, the patient learns to suppress that sudden severe urge to go to the toilet with the brain and gradually extend the hourly toilet habit to normal intervals of three hours.

How Do Medication Therapies Bring Urinary Incontinence Under Control?

When lifestyle modifications, dietary changes, and muscle-strengthening exercises alone do not provide sufficient improvement, pharmacological agents, that is, medication therapies, which are the next step of medical treatment, come into play. While medications form the backbone of treatment especially in sudden urge urinary incontinence and overactive bladder cases, their effectiveness is much more limited in stress incontinence that occurs with coughing and sneezing. These medications basically aim to correct the incorrect or excessive communication between the nervous system and the bladder muscle.

The main groups of medications used in the treatment of urge urinary incontinence are as follows:

Anticholinergics

Beta-3 adrenergic agonists

Desmopressin

Local estrogen creams

Anticholinergic medications, which have long been used as the gold standard in this field, block the special key points (receptors) on the surface of the bladder muscle that trigger contraction. In this way, they prevent the “contract” commands sent by the brain from reaching the muscle while the bladder is not yet full. The involuntary fluttering of the bladder stops, the storage capacity expands, and the patient can make it to the toilet much more comfortably. However, because the key points affected by these drugs are found not only in the bladder but also in the salivary glands and intestines, annoying side effects such as dry mouth or constipation may occur.

The newer generation Beta-3 agonists (for example, drugs containing the active ingredient Mirabegron) work through a different route. Instead of preventing contraction, they open the locks that enable the bladder muscle to actively relax during the storage phase and stretch like a wide sack. The biggest advantage of this method is that side effects such as dry mouth, which push patients to discontinue the medication, are much rarer; however, because they may have a slight increasing effect on blood pressure, careful monitoring is required in heart patients. In cases of bedwetting during sleep or frequent nighttime urination, hormone-like medications taken before bedtime that temporarily slow the kidneys’ nighttime urine production (Desmopressin) produce excellent results. In menopausal women, the urinary tract mucosa becomes thinner and drier due to estrogen deficiency; local estrogen creams applied to this area revitalize the tissues and increase water-tightness.

Contact us now to get detailed information about treatments and to make an appointment!

 Contact via WhatsApp

What Are the Surgical Methods for Urinary Incontinence in Women?

In female patients diagnosed with stress urinary incontinence, whose hammock-like support in the pelvic floor has physically completely collapsed and who have not benefited at all from months of exercise, surgical intervention is the most effective and definitive option to correct the anatomy. Modern surgical techniques today, unlike the major operations of the past in which the abdomen was opened with a large incision, are extremely delicate, minimally invasive, tissue-friendly methods with a very rapid recovery process.

Today, the most common and most successful method used for women worldwide is the “mid-urethral sling surgery,” in other words, “Sling” operations. The logic behind this surgery is to place a synthetic, completely biocompatible, mesh-like thin band exactly under the middle part of the urinary tract (urethra), which sags downward and opens during coughing or jumping. This band takes over the role of the weakened natural tissues and creates a new artificial hammock for the urinary tract. When intra-abdominal pressure suddenly increases, the urinary tract leans against this band and bends slightly, and the leakage of urine is completely prevented by a mechanical barrier.

Sling surgeries are divided into two according to their placement routes. In the TVT method, this thin band is passed upward just behind the pubic bone, while in the more frequently preferred TOT method, it is passed horizontally through the natural muscle spaces on the inner sides of the legs. Due to its anatomical structure, the TOT method is extremely safe because it reduces the risks of needle injury to the bladder or bowel damage almost to zero. This procedure, most often performed with a tiny one-centimeter incision made through the vagina, takes an average of twenty minutes, and patients can usually return home on the first day after surgery walking and, moreover, completely dry. Long-term success rates are above ninety percent. In patients who are too old to tolerate surgery or whose anesthesia risk is very high, bulking agents can be injected endoscopically into the inner surface of the urinary tract to narrow the channel; however, the permanence of these fillers is not as high as that of sling surgeries.

How Is Urinary Incontinence Treated in Men After Prostate Surgery?

Cases of stress urinary incontinence encountered in men are based on a completely different story from women. Since pelvic floor weakness is rare in male anatomy, the problem usually arises after major surgeries called radical prostatectomy performed for the treatment of prostate cancer. The prostate gland is a structure that surrounds the urinary tract right at the outlet of the bladder, and while this gland is completely removed during cancer surgery, the urinary holding muscle (external sphincter) just below it or the delicate nerve networks leading to this muscle may be damaged, even unintentionally. This condition may require a temporary recovery period or may turn into a permanent leakage that does not improve even after months.

For men experiencing such severe and persistent leakage, the most satisfying, most definitive, and ultimate solution offered by modern urology is the placement of an “Artificial Urinary Sphincter.” This technological marvel is an intelligent mechanism completely concealed inside the body, absolutely not visible from the outside, and consisting of three parts. The first part is a silicone cuff filled with fluid that surrounds the urinary tract (urethra). The second part is a pressure-regulating balloon placed behind the abdominal muscles. The third part is a small control pump placed inside the scrotum that the patient can feel with their fingers.

When the system is at rest, the cuff is filled with fluid and keeps the urinary tract tightly closed, allowing the person to remain completely dry. When the patient goes to the toilet and wants to urinate, they press the small pump in the scrotum lightly a few times with their fingers. This movement allows the fluid inside the cuff to be temporarily transferred to the balloon inside the abdomen. The cuff deflates, the urinary tract opens wide, and the person comfortably empties the bladder. About two to three minutes after urination is finished, the fluid automatically fills the cuff again thanks to the smart valves inside the device and safely closes the urinary tract once more. This technology is a surgical solution with incredibly high patient satisfaction, returning social life to even severe patients who use ten pads a day by making them completely dry. In men with milder leakage, “Male Sling” operations that support the urinary tract from below or adjustable-volume “ProACT” balloon systems are successfully applied.

What Advanced Treatments Are Applied in Cases of Urinary Incontinence That Do Not Respond to Medications?

For resistant patients who have been treated with medication for months with a diagnosis of Overactive Bladder, have tried different medication groups, but still cannot get rid of those sudden urgency attacks, the rush to the toilet, and the interruption of their nighttime sleep, the process does not end here. In cases where medications are insufficient or cannot be used due to side effects, urology specialists bring advanced interventional treatments such as neuromodulation and Botox applications into action.

Botulinum Toxin (Botox), which we know is frequently used in aesthetic dermatology to smooth wrinkles, is also a revolutionary savior in the treatment of urinary incontinence. This procedure is performed under operating room conditions, but mostly only under local anesthesia or very light sedation. With a thin illuminated camera called a cystoscope, the inside of the bladder is entered through the urinary tract. After the inner surface of the bladder is examined in detail, Botox medication is injected in millimetric doses into approximately twenty to thirty different points of the bladder muscle with the help of a special and very fine needle. Botox temporarily cuts the connection of those erroneous, excessive, and involuntary stimulation signals going to the bladder muscle; it calms it down as if unplugging the muscle. The effect of the procedure generally begins at the end of the first week, and patients notice that the sudden urgent feeling has disappeared. The duration of the effect varies between six months and one year depending on the person’s metabolism; when the effect diminishes, the procedure can be safely repeated.

Another advanced technology option is nerve stimulation techniques. Just like pacemakers that regulate rhythm disorders in the heart, there are systems that regulate the rhythm of the bladder. In PTNS, which is a simpler method, a very thin acupuncture needle is placed next to the tibial nerve that passes just inside the ankle, and very mild electrical signals are given. These signals climb upward along the leg, reach the bladder control center in the spinal cord, and slowly reprogram the brain’s faulty reflexes. In Sacral Neuromodulation (Bladder Pacemaker), which is a more permanent solution, very fine electrodes are permanently placed on the nerve roots in the tailbone region, and regular stimulation is sent to these nerves 24/7 with a chip/battery the size of a matchbox placed under the skin. This method is a wonderful option for permanently balancing bladder functions even in the most difficult cases resistant to all other treatments.

How Should Nocturnal Urinary Incontinence Seen in Childhood Be Approached?

Another condition that is as important and common as urinary incontinence problems in adults is bedwetting during sleep seen in children over the age of five, medically called nocturnal enuresis. This condition, which understandably worries families, actually usually stems from some timing delays in the child’s developmental process rather than an underlying anatomical disease.

If the child stays completely dry during the day and wets the bed only during deep sleep, this condition is called monosymptomatic enuresis. Here, the problem is generally that communication between the brain and the bladder does not fully function during sleep, the sleep is too deep to be awakened from, or the hormone (ADH) that slows nighttime urine production by the kidneys is not yet secreted at a sufficient level in the child. In these children, the first step is psychological support and the conditioning method called “Alarm Therapy.” A small sensor attached to the child’s pajamas beeps loudly when the first drop of urine falls and wakes the child; over time, the brain learns to wake up when the bladder is full or to hold the bladder. In medication treatment, dissolving tablets taken before bedtime that reduce the amount of urine are quite successful. However, if the child also has sudden urgency during the day, crosses their legs to hold the toilet, or leaks when laughing, this indicates a more complex bladder dysfunction and must definitely be treated with medications that calm the bladder muscle, just as in adults, after detailed tests are performed. During this process, the most critical point of treatment is for parents to display a patient, understanding, and non-blaming attitude toward the child.

How Can Individuals Experiencing Urinary Incontinence Take a Step Toward Healthy Living?

Urinary incontinence is a sneaky problem that imprisons the body and soul and separates a person from their loved ones, social life, and freedom. However, as we have examined in detail throughout this text, no patient has any obligation to live with this condition or tolerate it. Thanks to the diagnostic tools, harmless medication options, state-of-the-art smart devices, and minimal surgical techniques lasting twenty minutes available to the medical world, success rates are almost approaching one hundred percent.

Starting from small changes to be made in daily habits and extending to advanced surgical methods, in this wide range it is possible to create a personalized treatment plan suitable for each patient’s own biology, age, and life expectancy. What is important is to stop seeing this problem as a “normal cost” of old age or womanhood and take action. Returning to a dry and comfortable life where you can laugh freely, take long journeys without planning your route according to toilets, and live free from pads and worries is entirely in your hands.

#section_153346345 {
