The severity, duration, and accompanying symptoms of the pain vary depending on the underlying cause and require clinical evaluation.

Among the causes of groin pain in men, musculoskeletal strains are common and generally develop after heavy lifting or sudden movements. Sports injuries can create microtraumas in muscle fibers, leading to local tenderness and pain that increases with movement.

Urological diseases that cause groin pain include kidney stones, urinary tract infections, and testicle-related pathologies. These conditions usually progress together with additional symptoms such as a burning sensation, urinary changes, or tenderness in the testicles, and early diagnosis is important.

Other important conditions that cause pain in the groin area include inguinal hernia and enlarged lymph nodes. One of the most common of these situations is distinguishing inguinal hernia from urological groin pain.

Inguinal hernia occurs when intra-abdominal organs or fatty tissue protrude outward through a weak point in the abdominal wall. It is more common in men than in women. Inguinal hernia can cause a noticeable swelling in this area, a feeling of pressure, and pain. The pain especially increases during coughing, straining, or heavy lifting. However, these complaints can often be confused with groin pain caused by kidney stones, bladder stones, varicocele, or prostate problems.

The most important difference between urological pain and inguinal hernia is that hernia usually creates a swelling that can be felt by hand in the groin area. In addition, hernia pain is mostly triggered by movements that increase intra-abdominal pressure. In contrast, in urological causes such as kidney stones, the pain may start suddenly, spread from the flank toward the groin, and be accompanied by additional symptoms such as nausea and vomiting.

Infections such as prostatitis or epididymitis can also cause groin pain, but in these cases there are additional findings such as burning during urination, fever, or tenderness in the testicles. Therefore, evaluating groin pain alone can be misleading.

Since inguinal hernia and urological diseases show similar symptoms, detailed examination and necessary imaging tests are important for accurate diagnosis. Thus, unnecessary treatments are prevented and the real cause of groin pain can be determined.

Do Urinary Tract Infections Cause Groin Pain?

Urinary tract infections (UTIs), although less common in men than in women, can cause serious complaints. When infection develops in the kidney, bladder, urethra, or prostate area, pain reflected to the groin area may occur.

Bladder infection (cystitis) usually creates a feeling of discomfort in the groin area together with frequent urination, burning, and lower abdominal pain. In deeper infections, especially prostate inflammation (prostatitis), groin pain may be more pronounced and may be accompanied by systemic findings such as fever and chills.

In infections progressing to the kidneys (pyelonephritis), pain starting in the lower back may spread toward the groin. In addition, cloudy urine, bad odor, or the presence of blood in the urine seen during urinary tract infections are also among the typical symptoms accompanying groin pain.

If infections are not treated in time, the pain may become chronic and lead to more serious kidney problems. Therefore, when urinary complaints are seen together with groin pain, urinary tract infection must definitely be considered.

Does Prostate Cancer Cause Pain in the Groin?

Prostate cancer usually progresses silently in the early stage. In the later period, a tumor that extends beyond the prostate capsule may spread to surrounding tissues and cause pain in the groin area. This pain may often also be reflected to the lower back, hips, or legs. The cause of groin pain may be the prostate, but it cannot be said that this condition definitely indicates cancer.

The prostate is an organ of critical importance for the male reproductive and urinary tract system. It is located just below the bladder and surrounds the urinary canal, playing a role in the expulsion of both urine and semen from the body. Diseases related to this organ can cause not only urination problems but also pain and a feeling of discomfort in the groin area.

Prostatitis (prostate inflammation) is the most common prostate-related cause of groin pain. Acute prostatitis starts suddenly and causes severe groin and perineal pain, fever, burning during urination, and frequent urination. Chronic prostatitis, on the other hand, progresses more insidiously; it usually manifests with long-term, mild but disturbing groin pain, burning in the urine, and pain during ejaculation.

Benign prostatic hyperplasia (BPH) is benign enlargement of the prostate. Although it does not directly cause groin pain, it may create pressure-like discomfort in the groin due to a feeling of incomplete bladder emptying, frequent urination, and difficulty urinating. It is a common condition especially in older men.

Groin pain caused by prostate diseases may be confused with other urological disorders such as kidney stones or bladder infections. Therefore, it is necessary to make an accurate diagnosis with detailed examination, PSA testing, urinalysis, and imaging methods if necessary.

What Is the Cause of Sudden Groin Pain?

Sudden severe pain in the groin area in men may not always be a simple problem. One of the most urgent and vital causes of this pain is testicular torsion. Testicular torsion occurs when the vessels feeding the testicle twist around themselves and become compressed. Since blood flow is suddenly cut off, the testicle begins to be damaged in a short time.

In this condition, the pain usually starts suddenly and is very severe. It is mostly one-sided and is felt both in the testicle and in the groin area. Findings accompanying the pain include swelling in the testicle, redness, nausea, vomiting, and sometimes fever. In some patients, the pain may also spread to the lower back or lower abdomen.

Although testicular torsion can occur at any age, it is more common especially in adolescent males. The reason for this is that the testicles are anatomically more mobile during this period.

Time is very critical in this disease. The survival time of the testicle is generally limited to the first 6 hours. If surgical intervention is not performed within this period, the risk of losing the testicle is quite high. The diagnosis is mostly made by clinical examination, and ultrasound may be used in suspicious cases. However, surgery is usually performed directly to avoid losing time.

In treatment, the testicle is repositioned with surgical intervention and its blood supply is restored. In the same session, the other testicle is also fixed to reduce the risk of torsion in the future.

Does Varicocele Radiate to the Groin?

Varicocele is a vascular problem that occurs with the enlargement of the testicular veins. It is generally seen more often in the left testicle because anatomically the veins on this side are more sensitive to pressure. Varicocele is an important urological problem in men due to both its relationship with infertility and the discomfort it creates in the groin area.

Varicocele often does not cause obvious pain, but it creates a feeling of discomfort in the form of heaviness, pressure, or aching in the groin and testicular area. This feeling becomes more pronounced especially later in the day, when standing for a long time, or after intense physical activity. The pain is usually dull in character and may decrease with rest.

In some men, varicocele may go unnoticed, but in advanced degrees, the veins may become visible under the skin and an appearance described as a “bag of worms feeling” may occur. This discomfort felt in the groin and testicle may negatively affect daily life comfort.

One of the most important effects of varicocele is that it can impair sperm production. Sperm quality may decrease due to increased testicular temperature, and this may lead to infertility in men. Therefore, it is a disease that should be considered not only in terms of groin pain but also reproductive health.

The need for treatment varies from person to person. In mild cases, lifestyle adjustments and follow-up may be sufficient, while surgical treatment (microsurgical varicocelectomy) may be applied in those with significant pain or infertility problems.

Varicocele, seen together with a feeling of pressure and discomfort in the groin, is an important factor affecting both daily life and reproductive functions in men’s health.

Kidney Stone Pain

Kidney stones are one of the most common urological causes of groin pain. When a stone formed in the kidney falls into the ureter, the canal that carries urine to the bladder, it causes very severe pain. This pain usually starts in the lower back and spreads to the sides and groin area. In men, referred pain extending to the testicles may also often be seen.

Kidney stone pain is called renal colic in medicine and is a sudden-onset, severe pain that increases in waves. Patients describe this pain as a “stabbing” or “writhing” pain. This pain felt in the groin area is usually accompanied by nausea, vomiting, frequent urination, and blood in the urine.

The severity of the pain may vary depending on the size and location of the stone. Small stones sometimes pass spontaneously, while large stones can completely block the urinary tract and cause a serious increase in pressure in the kidney. This condition not only causes groin pain but also threatens kidney functions.

Groin pain caused by kidney stones may be confused with inguinal hernia or prostate problems. However, the typical feature of kidney stone pain is that it does not change with movement and generally follows a course spreading from the lower back to the groin. In addition, symptoms such as blood in the urine and frequent urination also provide clues in favor of kidney stones.

How Do Epididymitis and Orchitis Cause Groin Pain?

One of the urological causes of groin pain is infections affecting the testicle and surrounding structures. Epididymitis is inflammation of the sperm-carrying duct (epididymis) located at the back of the testicle. Orchitis means direct inflammation of the testicle. The two diseases may sometimes be seen together, and in this case the condition is called epididymo-orchitis.

Epididymitis usually occurs with bacterial infections. Sexually transmitted microorganisms (for example chlamydia and gonorrhea) are among the most common causes in young men. In older ages, urinary tract infections or prostate inflammation may spread to the testicles through the epididymis. Orchitis is mostly viral in origin; it can especially be seen after mumps disease in children.

The most obvious complaint in these types of diseases is pain in the groin and testicular area. The pain often gradually increases, and swelling, redness, and tenderness in the testicle accompany it. In epididymitis, burning in the urine, frequent urination, and fever may also be seen. In orchitis, especially high fever and general weakness are prominent.

Groin pain in these diseases develops both due to local inflammation and as a result of the spread of infection. The pain may become more pronounced while sitting or walking. Sometimes it may also spread to the lower back and lower abdomen.

The Role of Sexually Transmitted Diseases in Groin Pain

Sexually transmitted diseases do not only cause complaints in the genital area; they can also cause pain in the groin and lower abdomen. The most common sexually transmitted diseases in men include chlamydia, gonorrhea, trichomonas, and herpes infections. These infections may cause inflammation in the urinary canal, prostate, or epididymis, leading to pain in the groin area.

Bacterial infections such as chlamydia and gonorrhea usually manifest with burning during urination, discharge from the urinary canal, and frequent urination. In this case, the infection may progress upward and reach the epididymis, causing severe pain in the testicles together with the groin.

Herpes simplex virus creates painful sores in the genital area. Since these sores affect nerve endings, a burning, stinging, and constant feeling of discomfort may occur in the groin area. In parasitic infections such as trichomonas, groin pain usually appears together with urinary complaints.

Groin pain due to sexually transmitted diseases is mostly inflammation-related. Fever, weakness, swelling, or discharge from the urinary canal may accompany the pain. The pain may be one-sided or may be felt in both groin areas.

What Is Long-Lasting Groin Pain a Sign Of?

Groin pain often occurs due to causes such as stone diseases, prostate problems, or infections and improves when treated. However, in some men, the pain may continue for weeks or even months. In this case, one of the important causes that comes to mind is chronic pelvic pain syndrome.

Chronic pelvic pain syndrome is a condition characterized by pain in the groin, perineum, and lower abdomen lasting at least three months with no other cause found. It is generally associated with the sensitivity of nerves and muscles around the prostate. Therefore, it may sometimes be mistakenly considered chronic prostatitis.

The pain is often in the form of a dull and constant discomfort. A feeling of pressure in the groin, burning during urination, frequent urination, and pain during sexual intercourse may accompany it. Some men also describe the pain as spreading to the lower back and thigh. Although symptoms vary from person to person, the most prominent feature is that the pain continues for a long time and affects daily life.

Although the causes of chronic pelvic pain syndrome are not fully known, it is thought that previous urinary tract infections, immune system response, nerve sensitivity, and psychological factors play a role together. It is also seen that stress and anxiety can increase the severity of pain.

What Causes Swelling and Pain in the Groin?

Swelling and pain in the groin can be seen in men for different reasons. Two of these causes are spermatocele and hydrocele developing around the testicle. These conditions, which are mostly benign, may cause discomfort and pain in the groin depending on size and the amount of fluid accumulation.

Spermatocele is a fluid-filled cystic structure that forms in the epididymal duct located at the back of the testicle. When small in size, it is usually not noticed and does not cause pain. However, as the cyst grows, fullness in the testicle and a feeling of pressure reflected to the groin may occur. After standing for a long time or heavy activities, this discomfort may appear more prominently in some men.

Hydrocele occurs with fluid accumulation around the testicles. It may be congenital in newborns or may develop in adults after trauma, infections, or surgery. Hydrocele is often painless, but it can create discomfort in the groin area by causing swelling and tension in the scrotum. If the amount of fluid increases, groin pain may become more pronounced while walking or sitting.

In both conditions, swelling usually increases gradually and can be felt by hand. Diagnosis is mostly made easily with physical examination and ultrasonography. While small and asymptomatic cases are followed, surgical treatment is preferred in large and uncomfortable cases.

Spermatocele and hydrocele are usually harmless, but when they cause swelling and pain in the groin, they can affect daily life comfort.

Groin pain in men occurs due to various causes such as muscle strains, hernia formation, infections, and urological diseases.
