After Prostate Surgery?

The possibility of regrowth after prostate surgery depends on the surgical method applied and the patient’s individual health condition.

In TUR-P (Transurethral Resection of the Prostate), one of the most commonly performed methods, and laser operations, only the part blocking the urinary tract is removed. In other words, the entire prostate is not removed; only the section that has enlarged and caused problems is cleared. Therefore, there is a possibility that the remaining prostate tissue may grow again over time. This growth usually occurs gradually over the years, and the patient may begin to experience complaints such as frequent urination again, incomplete emptying of the bladder, or weakening of the urine flow.

In open prostatectomy, which is performed in more advanced cases, a large part of the prostate tissue is removed. Therefore, the risk of regrowth is quite low compared to TUR-P. However, no surgical method guarantees that the prostate will remain the same for life. Especially with advancing age, prostate tissue may redevelop due to hormonal changes.

Radical prostatectomy, preferred for prostate cancer, has a different approach from other surgical methods. In this method, the entire prostate gland, some surrounding tissues, and, when necessary, lymph nodes are removed. In this case, benign prostate enlargement does not recur because the prostate gland no longer exists. However, when prostate cancer is involved, the risk is different: if cancer cells have spread outside the prostate, the disease may recur. Therefore, patients who have undergone radical prostatectomy must have regular PSA tests and should not neglect their oncological follow-ups.

The risk of regrowth after prostate surgery does not disappear completely. The level of this risk depends on the type of surgery, the patient’s age, hormonal balance, and lifestyle. Regular doctor check-ups and healthy lifestyle habits are of great importance in minimizing the possibility of regrowth.

Side Effects That May Occur After Prostate Surgery

Although prostate surgery is generally a safe operation, some side effects and complications may occur, as with any surgical procedure. These effects may be short-term or, rarely, permanent.

One of the most common problems is urinary complaints. After surgery, patients may experience frequent urination, burning during urination, sudden urgency, or temporary urinary incontinence. These symptoms mostly disappear within a few weeks as the bladder adapts to the new condition. Exercises that strengthen the pelvic floor muscles, especially Kegel exercises, are quite beneficial during this period.

Another important side effect is changes related to sexual health. After surgeries such as TUR-P, the most common condition is “retrograde ejaculation,” meaning semen goes into the bladder instead of coming out. Although this is not harmful to health, it may be a problem for patients planning to have children. In radical prostatectomies, erection problems may occur; this condition usually improves over time but may be permanent in some patients.

More rarely, bleeding, urinary tract infection, and narrowing of the urinary canal (urethral stricture) may develop. Regular doctor check-ups are important for early detection of such complications.

Do Urinary Problems Recur After Prostate Surgery?

Most urinary complaints decrease after prostate surgery. However, in some patients, urinary problems do not disappear completely or may reappear over the years. The reason for this is not only regrowth of the prostate; weakening of the bladder muscles or narrowing of the urinary tract may also cause similar problems.

Urinary problems seen in the first weeks after surgery are usually temporary. Burning during urination, urgency, dribbling, or temporary urinary incontinence are the most common complaints. As the bladder adapts to the new condition, these problems largely disappear. Pelvic floor exercises are especially effective in this process.

In the long term, regrowth of the remaining prostate tissue may cause urinary problems to return. This is especially the case with TUR-P or laser methods because only the obstructive part is removed in these operations. The risk is lower in open surgeries. In addition, as age advances, loss of strength in the bladder muscles may also occur, and this may prevent complete emptying of urine.

The way to minimize these risks is not to neglect regular check-ups and to pay attention to lifestyle. Healthy nutrition, adequate water consumption, and staying away from smoking positively affect bladder health.

Does Surgery Completely Eliminate the Risk of Prostate Cancer?

In radical prostatectomy performed due to prostate cancer, the entire prostate gland, some surrounding tissues, and, if necessary, lymph nodes are removed. This procedure aims to eliminate cancer. Surgery is highly successful in patients diagnosed at an early stage, and the likelihood of cancer returning is low. However, the situation may be different in advanced cases.

If the cancer has exceeded the prostate capsule and spread to surrounding tissues or lymph nodes, surgery alone may not be sufficient. In such a case, tumor cells may remain in the body and multiply again over time, causing the disease to recur. Therefore, regular follow-up after surgery is also of great importance.

The most commonly used method during follow-up is the PSA (Prostate-Specific Antigen) test. A decrease in PSA value indicates that the surgery was successful. However, a rise in values again may indicate that the cancer has returned. In such a case, additional methods such as radiotherapy or hormone therapy may be applied.

The Effect of Lifestyle and Nutrition on Recurrent Prostate Problems

Whether prostate problems recur after surgery does not depend only on the surgery performed; the patient’s lifestyle and nutritional habits also play a major role in this process. A healthy lifestyle both reduces the possibility of prostate regrowth and preserves bladder functions.

Diet is critically important at this point. A diet rich in vegetables, fruits, whole grains, fish, and healthy fats such as olive oil supports prostate health. Especially foods rich in antioxidants, such as tomatoes, broccoli, and cauliflower, have a protective effect on prostate tissue. In contrast, excessive red meat, processed foods, fatty foods, and fast food-type foods increase the risk of recurrence of prostate problems.

Fluid consumption should also be balanced. Drinking enough water throughout the day cleanses the urinary tract and helps the bladder function healthily. However, excessive fluid consumption in the evening should be limited, as it may increase frequent urination at night. In addition, alcohol and caffeinated drinks may irritate the urinary tract and increase complaints.

Exercise and weight control are also important. Regular walking, swimming, or light sports activities strengthen blood circulation, reduce the risk of obesity, and support bladder muscles. Smoking should be stopped because it harms both prostate health and the general vascular structure.

The Importance of Regular Check-Ups After Prostate Surgery

Prostate surgery provides permanent relief in many patients, but the treatment process does not end with surgery. Regular doctor check-ups are essential for healthy recovery in the long term. These check-ups allow complications to be detected early and minimize the risk of prostate regrowth.

One of the most important evaluation methods during check-ups is the PSA (Prostate-Specific Antigen) test. This test is especially important in patients who have undergone surgery due to prostate cancer. Low PSA levels indicate that the disease is under control. An increase in values may indicate the possibility of recurrence.

In patients who have undergone surgery for benign prostate enlargement, doctors usually check bladder functions with tests such as urinalysis, ultrasound, and uroflowmetry. In this way, conditions such as narrowing in the urinary tract, incomplete emptying of the bladder, or growth of the remaining prostate tissue can be noticed at an early stage.

Regular check-ups are also important in managing side effects that may occur after surgery. Problems such as urinary incontinence, erection problems, or infection can be treated much more easily when diagnosed in time.

Do Sexual Health Problems Recur in Those Who Have Had Prostate Surgery?

Sexual health-related problems after prostate surgery are among the topics patients are most curious about. Different effects may be seen depending on the type of surgery, the patient’s age, general health condition, and whether the nerves were preserved during surgery.

One of the most common problems is erectile difficulty. In more extensive surgeries such as radical prostatectomy, the nerves that provide erection may be damaged during the operation. While this condition is temporary in some patients, it may become more permanent in others. In TUR-P or laser methods, erection problems are rarer.

Another common problem is retrograde ejaculation. In this condition, especially seen after TUR-P, semen goes into the bladder instead of coming out. Although it is not dangerous for health, it is an important problem for men who are considering having children.

Decreased sexual desire is generally related to psychological factors. Stress, anxiety, and performance concerns after surgery may negatively affect sexual desire. Psychological support, communication with the partner, and medication treatments when necessary help at this point.

The positive side is that most of these problems are not permanent. Over time, a large part of sexual functions can be regained through nerve healing, pelvic floor exercises, a healthy lifestyle, and medical treatments.

Most Commonly Performed Types of Prostate Surgery

There is not a single method for prostate surgery. Which technique will be applied is determined according to the size of the prostate, the patient’s general health condition, age, and the severity of complaints. Today, both classical methods and modern, minimally invasive techniques are used.

TUR-P (Transurethral Resection of the Prostate): It is the most commonly used method. A special device is inserted through the urinary tract, and the part of the prostate blocking the urinary canal is removed. It is less invasive than open surgery, patients’ hospital stay is shorter, and most patients can return to daily life in a short time.

Open Prostatectomy: It is preferred in very large prostates or in cases accompanied by additional complications. A large part of the prostate is removed through an incision made in the lower abdomen. Recovery time is longer, but it is an effective solution for severely enlarged prostates.

Laser Methods (HoLEP, GreenLight): In these methods, which have become increasingly popular in recent years, prostate tissue is burned, cut, or vaporized with laser energy. Lower risk of bleeding, rapid discharge, and short recovery time are the most important advantages. It stands out as a safe option especially in patients using blood-thinning medications.

Robotic Prostatectomy: It is a modern method especially preferred in prostate cancer. This surgery, performed with robotic assistance, allows the surgeon to make more precise movements. Since the likelihood of preserving the nerves is higher, it is advantageous in terms of preserving sexual functions after surgery.

Water Vapor Method (Rezūm): It is one of the new minimally invasive methods. Water vapor is injected into the prostate tissue, destroying the cells, and the prostate shrinks over time. It can be performed under local anesthesia and usually does not require hospitalization. Its side effects are fewer compared to other surgeries. It is a good alternative for elderly patients or those who are not suitable for general anesthesia.

What Happens If the Prostate Gland Is Completely Removed?

When the prostate gland is completely removed, some important changes occur in the male reproductive and urinary systems. This procedure is usually performed with a surgical operation called radical prostatectomy and is most commonly applied in the treatment of prostate cancer.

First, there is a permanent effect on fertility. The prostate produces the fluid part of semen and provides secretions that nourish sperm and facilitate their movement. When the prostate is completely removed, this fluid cannot be produced. Therefore, the patient can no longer ejaculate semen and the chance of having children naturally disappears.

The second important effect is on sexual functions. Very sensitive nerves that control erection are located around the prostate. These nerves may be damaged during surgery. This may lead to erection problems. Nerve-sparing surgical techniques used today can reduce this risk but cannot completely eliminate it. Therefore, some patients may experience permanent or temporary erection problems.

Thirdly, urinary control may be affected. After the prostate is removed, the muscles that ensure urine retention may initially be strained. Many patients may experience temporary urinary incontinence in the postoperative period. However, thanks to regular pelvic floor (Kegel) exercises, this problem usually improves within a few months. Permanent urinary incontinence may occur in a very small number of patients.

Complete removal of the prostate is one of the most effective methods for preventing the spread of prostate cancer. Especially if the cancer is detected at an early stage, the patient’s life expectancy increases after surgery and the disease is largely brought under control.

In individuals who have undergone prostate surgery, experiencing prostate problems again may vary depending on the surgical method applied and the type of disease.
